About the Role
When families call us, it’s often during one of the most difficult and uncertain moments of their lives.
As an Admissions Coordinator, you will be the first voice they hear—the one who brings calm, clarity, and compassion when it matters most. This role is critical in ensuring families feel supported from the very first interaction while helping guide them through the initial steps of hospice care.
We are seeking someone with prior healthcare experience—ideally in hospice, home health, or a related setting—who understands both the emotional and operational aspects of patient care coordination.
This role requires someone who is not only organized and efficient, but who can confidently navigate sensitive conversations, communicate clearly, and coordinate seamlessly with clinical teams.
What You’ll Do
- Answer incoming calls and provide a warm, compassionate, and professional first impression
- Serve as the initial point of contact for patients, families, and referral sources
- Guide callers through the first steps of hospice services with clarity and confidence
- Gather essential information and ensure timely coordination of next steps
- Communicate effectively with clinical staff, intake team, and referral partners
- Support a smooth, efficient, and patient-centered admissions process

What We’re Looking For
- Previous experience in a healthcare setting required (hospice, home health, hospital, or similar)
- Hospice or end-of-life care experience strongly preferred
- Strong communication skills with a professional and compassionate phone presence
- Ability to work in a fast-paced, team-oriented environment
- Experience coordinating with clinical teams or handling referrals is a plus

Requirements
Education & Experience
- High school diploma or equivalent required
- Associate’s or Bachelor’s degree preferred
- Minimum 1–2 years of experience in a healthcare, hospice, or patient-facing administrative role
- Experience with hospice, home health, or care coordination strongly preferred
