Description

 

We are seeking a highly skilled, detail-oriented, and experienced Quality Assurance (QA) Nurse to join our home health team. In this role, you will play a critical part in ensuring the delivery of safe, compliant, and high-quality patient care in accordance with Medicare Conditions of Participation (CoPs), agency policies, and best clinical practices.

The QA Nurse will work closely with clinical leadership and field staff to monitor performance, identify opportunities for improvement, and support continuous quality improvement across the agency

Responsibilities:

Conduct regular quality assurance audits of patient records, care plans, and clinical documentation

Perform OASIS record reviews according to agency-defined parameters and required timeframes

Review and validate 12–16 OASIS assessments per day for accuracy, completeness, and compliance (no coding required)

Identify trends, deficiencies, and risk areas, and develop corrective action plans

Collaborate with nursing and interdisciplinary team members to ensure compliance with clinical standards, regulations, and agency policies

Provide education, feedback, and training to nursing staff related to documentation quality and regulatory compliance

Participate in interdisciplinary team meetings to discuss patient care quality and improvement opportunities

Maintain accurate, organized, and complete records of all QA activities and findings

Demonstrate thorough knowledge of current Medicare guidelines and Home Health Conditions of Participation (CoPs)

Support agency preparedness for surveys, audits, and regulatory reviews

Requirements

 

Active and unrestricted Registered Nurse (RN) or LPN license

Home health experience required

Strong experience with OASIS review and documentation compliance required

Proven ability to consistently meet productivity expectations of 12–16 OASIS reviews per day

In-depth knowledge of Medicare home health regulations and CoPs

Excellent organizational, analytical, and communication skills

Ability to work independently while collaborating effectively with a multidisciplinary team

Detail-oriented with a strong commitment to quality patient care
