Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking a Primary Care Physician in Portsmouth, Virginia. This is a full-time outpatient opportunity within a value-based care model focused on adult and geriatric populations, offering strong compensation, clinical support, and meaningful patient relationships.

POSITION OVERVIEW
This role allows physicians to practice patient-centered medicine without RVU pressure. You will manage a smaller patient panel, focus on preventive care, and work within a highly collaborative, team-based environment designed to improve outcomes for older adults.

SCHEDULE
• Monday through Friday
• 8:00 AM to 5:00 PM
• Outpatient-only setting

COMPENSATION
• Base Salary: $235,000 – $270,000
• Annual Bonus: Up to 20% of base salary
• Sign-On Bonus: Up to $40,000+ (based on experience)
• Total First-Year Compensation: $300,000+
• Relocation assistance available

KEY RESPONSIBILITIES
• Provide comprehensive primary care to adult and geriatric patients
• Manage chronic conditions and preventive care plans
• Conduct wellness visits and health risk assessments
• Collaborate with interdisciplinary care teams
• Maintain accurate and compliant EMR documentation
• Drive patient engagement and improved health outcomes

CARE MODEL
• Outpatient primary care focused on Medicare population
• Value-based care model emphasizing outcomes
• Integrated care team including NP/PA, MA, social work, and pharmacy
• Focus on prevention, chronic care management, and patient relationships

• MD or DO required
• Board Certified or Board Eligible in Internal Medicine or Family Medicine required
• Active, unrestricted medical license required
• Active DEA license required
• U.S. work authorization required

• Value-based care experience preferred but not required
• Geriatric experience preferred
• HEDIS experience preferred
• Strong documentation skills required
• Strong team-based care mindset
