We are seeking a dedicated and compassionate *PRN* Certified Registered Nurse Practitioner (CRNP) to join our hospice care team. In this role, you will be a vital member of the Interdisciplinary Care Team (IDT), working collaboratively with the Medical Director, physicians, and other healthcare professionals to provide exceptional, patient-centered hospice care.

This position is ideal for a highly motivated and organized practitioner who thrives in a collaborative environment and is passionate about improving the quality of life for patients and their families.


What You’ll Do

As a CRNP in our hospice program, you will:

  • Provide comprehensive medical care and assessments in both facility and home settings 
  • Perform physical examinations and preventive care under physician guidance 
  • Order, interpret, and evaluate diagnostic tests to assess patient conditions 
  • Develop and document treatment plans, findings, and prognoses 
  • Prescribe medications and recommend treatments as appropriate 
  • Participate in ongoing care coordination and case discussions 
  • Ensure compliance with Medicare Face-to-Face Encounter requirements

What You Bring (Qualifications)

  • Graduate of an accredited Nurse Practitioner program 
  • Current national certification as a CRNP 
  • Strong knowledge of hospice care principles and practices 
  • Familiarity with Medicare Face-to-Face Encounter criteria 
  • Excellent clinical assessment, diagnostic, and treatment skills 
  • Ability to work effectively within an interdisciplinary team 
  • Outstanding communication and organizational skills 
  • A genuine passion for compassionate, patient-focused care
