About the Role
Touchstone Home Health is seeking a Speech Language Pathologist (SLP) to join our San Antonio team—where clinical excellence meets purpose-driven care. In this role, you’ll deliver skilled, compassionate therapy that helps patients regain communication, independence, and quality of life—all in the comfort of their homes.

Key Responsibilities

  • Provide speech therapy services in accordance with physician orders and individualized care plans 
  • Conduct comprehensive evaluations of communication and swallowing needs 
  • Develop and adjust treatment plans to support patient progress and safety 
  • Educate patients, families, and caregivers on home-based strategies and exercises 
  • Complete timely and accurate documentation, including OASIS assessments and care updates 
  • Collaborate with physicians and interdisciplinary teams to coordinate high-quality care
Requirements

What We’re Looking For

  • Master’s or Doctoral degree in Speech Language Pathology 
  • Current SLP license in the state of Texas (or eligibility) 
  • Strong communication skills and the ability to connect with diverse patients and families 
  • Adaptability in dynamic, home-based care environments 
  • Organized, self-directed approach to managing schedule and documentation 
  • Willingness to travel between patient homes throughout the San Antonio area
