Behavioral Health Case Manager
Description

ESSENTIAL JOB RESPONSIBILITES:

  • Assesses for immediate and long-term service needs
  • Completion of monthly observation and monitoring.
  • Conducts and Completes Treatment Plans.
  • Assist residents with navigating social service systems and accessing benefits, including Social Security (SSI/SSDI) and other relevant programs.
  • Obtain and maintain SOAR (SSI/SSDI Outreach, Access, and Recovery) certification.
  • Planning, scheduling, and coordinating monthly meetings with residents and stakeholders.
  • Responding to client changes in needs, completing/ submitting appropriate paperwork.
  • Researching, accessing, and linking to services.
  • Communicating and working with providers to ensure appropriate service provision.
  • Meeting with supervisor for weekly supervision and consultation.
  • Attending/Participating in staff meetings with RA’s.
  • Assist with formal and informal training of residential staff according to policy and funding source as needed.
  • Monitor the quality of residential programs through established reviews.
  • Complete application for social security and other benefits.


ACCOUNTABILITIES:

I. PROVISION FOR CASE MANAGEMENT SERVICES:

A. Assesses and addresses in ISRP/needs expressed.

B. Make referrals to appropriate agencies and/or providers.

C. Documents and follows up in a timely manner to ensure services are in place and appropriate

D. Establish ongoing rapport with clients and assists them as appropriate in maintaining community living.

E. Responds to client referrals and needs in a timely manner.

F. Empower clients and families to be active participants in their care.

G. Works with the client, service providers and families in setting long term and short term goals for the client; link services to meet these goals; and monitors progress of the client achieving set goals.

H. Follow up on all benefits applied for the residents 


II. CLIENT FILE MAINTENANCE

A. Keep accurate and up-to-date records and documentation.

B. Completes Comprehensive Plan of Care and quarterly reviews, amendments and other paperwork.

C. Maintains thorough, up-to-date progress reports. 

 

III. PROVIDES ASSISTANCE OR FILLS IN FOR OTHER CASE MANAGERS AS NEEDED AND/OR DIRECTED BY SUPERVISOR.


IV. STAFF DEVELOPMENT

A. Work with the Residential Supervisor to ensure maximum communication and coordination with other Easterseals staff, e.g., administrative assistants, other program development staff, etc.

B. Work with Residential Supervisor and RA’s in evaluating program effectiveness and compliance within State/Contract requirements and program goals.

C. Participate in training activities and programs as required by funding source, policy, and supervisor.

D. Participate in trainings for certifications, credentials or specialties as determined appropriate by Easterseals Louisiana, funding source or supervisor. 

E. Provides input and assistance to Residential Supervisor in the preparation of program reports.

H. Building Relationships – Able to establish rapport with provider agencies and staff, Easterseals administrative and other program staff. 

I. Team Playing – Work cooperatively with other Easterseals Residential Staff/Supervisors. 

J. Mission Advocacy – Ability to identify with project and Easterseals objectives; adequately, constructively and directly communicate these objectives. 

Requirements

QUALIFCATIONS:

  • Must have a bachelor’s degree in social work or human services-related field and one year experience in the social service field. 
  • Must have own transportation. 
  • Must have excellent written and verbal communication skills. 
  • Must be a team player.
  • Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
  • Will be required to pass Easterseals Louisiana background check.


PHYSICAL REQUIREMENTS with or without reasonable accommodations:

  • The employee is regularly required to operate a computer, file and retrieve written documents, communicate with others on the phone and in-person. 
  • The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items. 
  • Ability to move independently within the facility and community. 
  • Must be able to travel and meet with participants 

 

WORKING CONDITIONS:

 Work is performed in a single-story residential setting that is environmentally controlled in Monroe, Louisiana.
