What You’ll Do:
As a key clinical leader, you will provide comprehensive primary care services, including diagnosis, treatment, prevention, and care coordination. You’ll work closely with physicians and interdisciplinary team members to ensure high-quality, patient-centered care.

· Perform patient assessments, histories, and physical exams 

· Diagnose conditions and develop individualized treatment plans 

· Prescribe medications in accordance with NYS guidelines 

· Perform minor procedures and manage chronic conditions 

· Review labs, imaging, and patient communications with timely follow-up 

· Coordinate referrals and continuum of care needs 

· Participate in quality initiatives, outreach programs, and team collaboration 


What You Bring:

· Active New York State NP license and DEA certification 

· Board Certified (or eligible, with certification within 18 months) 

· Strong clinical judgment and commitment to evidence-based care 

· Passion for community health and team-based care

Salary Description
$125,000-$170,000
