We are looking for a compassionate, organized, and clinically strong Resident Care Manager (RN) to join our team. In this role, you will help oversee resident care, coordinate clinical services, support interdisciplinary communication, and ensure residents receive high quality, person centered care.

What You’ll Do
• Oversee and coordinate resident care planning and clinical services
• Conduct resident assessments and help develop individualized care plans
• Monitor resident conditions and communicate changes with physicians, providers, and interdisciplinary team members
• Review physician orders, test results, and treatment plans to ensure continuity of care
• Utilize electronic medical records to document and maintain accurate clinical information
• Communicate with residents and families regarding care plans, concerns, and updates
• Participate in clinical meetings and collaborate with nursing leadership and other departments
• Provide education and support to residents, families, and staff regarding care needs and treatment plans
• Support smooth admissions, transitions of care, and discharge planning processes
• Ensure clinical documentation is accurate, complete, and compliant with regulations and facility standards
• Assist in supporting and guiding nursing staff while promoting a positive team environment
• Use strong clinical judgment and critical thinking to help provide safe, high quality resident care
• Follow all federal, state, facility, and company regulations and nursing standards
• Help maintain a positive, supportive, and resident focused culture throughout the facility
What We’re Looking For
• Current RN license in the State of Oregon
• Valid CPR certification
• Strong clinical assessment and critical thinking skills
• Excellent communication, leadership, and organizational abilities
• Compassionate and resident centered approach to care
• Ability to multitask and stay organized in a fast paced environment
• Strong documentation and care coordination skills
• Ability to work collaboratively with residents, families, providers, and interdisciplinary teams
• Dependable attendance and professionalism
• Long term care or skilled nursing experience preferred
• Supervisory or leadership experience preferred

Education
• Graduate of an accredited School of Nursing
• BSN preferred
Licenses/Certifications
• Current RN Licensure in the State of Oregon
• Valid CPR Certification
Experience
• Six months of experience in a long term care environment preferred
• Minimum of one year of supervisory or leadership experience in a healthcare setting preferred
