Description
Under the direction of the RN Case Manager/Chief Nursing Officer, perform discharge planning by assisting the Physician in arranging post discharge care prior to discharge. Ensure the safe discharge plan prior to discharge from the hospital. The Discharge planner communicates effectively and demonstrates a positive attitude and customer services skills with physicians, staff, and patients.
Requirements
- Current Licensure in California as a Licensed Vocational Nurse or graduate of accredited School of Medicine/Nursing
- Experience in health care delivery system with focus on Utilization Review / Case Management
