Description

  

The Licensed Practical Nurse is responsible for assisting all providers with patient care in the clinical aspect of the practice. The LPN will prepare wound care patients for clinic visits and examinations, assist the provider in all aspects of patient care, provide patient instruction, document and maintain the clinical records, and maintain supply inventory. The LPN works under direct supervision of the RN(s) or Providers.

Responsibilities: 

  • Supporting      the Center’s Mission Statement
  • Setting      up and maintaining clinical supplies
  • Assisting      all providers with patient care and minor procedures in clinic
  • Checking      supplies, solutions, and medications daily and discarding expired products      as needed
  • Utilizing      judgement in setting priorities and logical sequence and timing of major      tasks, duties, and procedures
  • Organizing      and cleaning exam rooms and setting up patient for proper physician      functioning. Storing supplies and linens, checking for defects and expired      supplies
  • Disposing      of soiled linens and hazardous waste according to infection control      guidelines
  • Demonstrating      strong communication skills and timeliness in answering telephone      messages, placing calls, and relaying messages to patients, providers, and      other staff
  • Preserving      patient's rights and privacy through maintaining confidentiality in      communications
  • Adhering      to all policies and procedures while providing safe patient care
  • Assuming      responsibility in performing other assigned duties, such as all types of      wound care, proper documentation, sending specimens to the lab and      assisting patient care, including vital signs and non-narcotic medication      refills as ordered by the provider. 
  • Maintaining      patient safety and protection from injury by proper utilization of safety      devices, equipment, and supplies
  • Maintaining      personal safety and protection from injury through proper utilization of      protective equipment
  • Utilizing      proper body mechanics when lifting supplies
  • Coordinating      functions in the Center to help maintain smooth operations
  • Communicating      effectively with other staff members and providers in the Center
  • Answering      telephone calls and directing calls to the appropriate staff
  • Requisitioning      supplies and maintenance repair services as directed through utilization      of the proper individuals
  • Organizing      Center and Clinic paperwork and supplies on a daily basis, and performing      any administrative duties required· Obtaining supplies from other areas as      needed and returning supplies and equipment to appropriate locations in a      timely manner
  • Organizing      and scanning Center charts and paperwork in a timely manner according to      policy
  • Assisting      in keeping rooms clean, stocked, neat, and orderly
  • Effectively      handling health or safety hazards for self and patients
  • Reporting      and documenting any unusual occurrences or adverse incidents accurately to      the provider and/or Clinical Nurse Manager
  • Performing      administrative and clerical duties as required
  • Ability      to travel to satellite clinics on a weekly and as needed basis with      providers to assist with procedures and patient care· Proper and timely      documentation of all patient care and phone calls in HER according to      policy, as well as confirming/maintaining past medical and surgical      history, and current medication and allergy lists
  • Communicating      all test results to providers and documenting appropriately.

Wound Care Focused Responsibilities:

Under the direction of the RN or Provider. Responsible for Patient Intake and Discharge to include but not limited to:

  • Medical      and surgical history documentation
  • Medication/Allergies      documentation
  • Social      history documentation
  • Nutritional        status documentation
  • Dressing      removal and wound cleansing
  • Measurement      of wounds
  • Reapplying      dressings including multi-layer wraps, and NPWT
  • Assists      with case management under the direction of RN, in accordance with state      regulations
  • Reports      quality of care issues to Clinical Nurse Manager.
  • Communicates      as needed with all team members regarding therapeutic interventions, to      ensure quality of care.
  • May      function as a Documentation Assistant (scribe) 

Performs  other duties as delegated 

  

Minimum Qualifications: 

  • High      School Diploma or General Education Development (GED)
  • Graduate      from an accredited practical nursing program/state approved educational      program
  • Current      Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license      in the state of practice -to maintain throughout the duration of      employment in the position
  • Two or      more years’ experience as LPN/LVN, to include one (1) or more      years in SNF or LTACH setting
  • Wound      care experience preferred
  • Must      possess flexibility and be able to organize and set priorities for work to      be done in a timely and efficient manner
  • Must      be able to understand and follow instruction with speed and accuracy
  • Maintain      current BLS Certification and completion of initial and annual required      Inservice training
  • Must      possess a sense of responsibility, dependability, efficiency, initiative,      accountability, and honesty
