As a Home Health RN Case Manager, you will coordinate and deliver individualized care to patients in their homes. You’ll work independently while collaborating with a supportive interdisciplinary team, helping patients recover, maintain independence, and achieve their health goals.Your role is essential in helping patients feel safe, supported, and confident—right where they live.

  • Develop and manage personalized care plans based on each patient’s unique needs
  • Perform comprehensive assessments and ongoing evaluations
  • Provide skilled nursing care, treatments, and patient education
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Support patients and families in understanding diagnoses, medications, and care plans
  • Monitor progress and adjust care to ensure the best outcomes
  • Document care accurately and meet all regulatory standards
  • Build meaningful, trusting relationships with patients and their families

Help bring care, comfort, and compassion home to those who need it most. You'll need:

  • Active RN license in the state of practice
  • Previous home health or case management experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently while thriving in a team environment

Requirements

JOB REQUIREMENTS: 

  • RN or BSN degree from an accredited College of Nursing, Nurse Licensure as a registered nurse in Iowa. 
  • One year clinical experience preferred; or . 
  • Home care or community health nursing experience preferred. 
  • Car, driver's license and adequate insurance as outlined in the personnel policies. 
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.



Salary Description
$79,040.00 - $83,200.00
