As a Home Health RN Case Manager, you will coordinate and deliver individualized care to patients in their homes. You’ll work independently while collaborating with a supportive interdisciplinary team, helping patients recover, maintain independence, and achieve their health goals.Your role is essential in helping patients feel safe, supported, and confident—right where they live.
- Develop and manage personalized care plans based on each patient’s unique needs
- Perform comprehensive assessments and ongoing evaluations
- Provide skilled nursing care, treatments, and patient education
- Coordinate care with physicians, therapists, and other healthcare professionals
- Support patients and families in understanding diagnoses, medications, and care plans
- Monitor progress and adjust care to ensure the best outcomes
- Document care accurately and meet all regulatory standards
- Build meaningful, trusting relationships with patients and their families
Help bring care, comfort, and compassion home to those who need it most. You'll need:
- Active RN license in the state of practice
- Previous home health or case management experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently while thriving in a team environment
- A compassionate, patient-centered mindset

JOB REQUIREMENTS:
- RN or BSN degree from an accredited College of Nursing, Nurse Licensure as a registered nurse in Iowa.
- One year clinical experience preferred; or .
- Home care or community health nursing experience preferred.
- Car, driver's license and adequate insurance as outlined in the personnel policies.
