Sylva Valley Health & Rehab is hiring Licensed Practical Nurses (LPNs) for Day shift (7a–7p).

Responsibilities

Provide direct nursing care per physician orders and care plans
Administer medications and treatments safely and accurately
Monitor residents and report changes in condition
Document care clearly and timely
Collaborate with CNAs, nurses, providers, and the interdisciplinary team
Maintain a safe, respectful, resident-centered environment

Requirements

Active Licensed Practical Nurse (LPN) license
Skilled nursing / long-term care experience preferred
Strong clinical skills and dependable attendance
Team-oriented mindset
