CPIhealth is seeking a highly organized and patient-focused Case Manager to serve as a liaison between providers and patients to ensure seamless coordination of care. This will be accomplished by scheduling patients at point of care after the initial clinic physician visit per guidelines set by CPIhealth, identify patients based on risk and behaviors for participation and manage the CPIhealth’s restricted opioid management protocol, monitors patients keep scheduled appointments and works closely with providers on missed appointments. This position is expected shadow the Clinic Physician’s schedule and be physically located where the Clinic Physician is located
Essential Functions:
All Patients:
- Serves as a liaison to providers and patients for coordination of services.
- Schedule patient’s follow up visit after initial new patient visit with appropriate APP or IPM provider based on the guidelines set by CPIhealth.
- Ensures fair distribution of visits as per management oversight and direction.
- Monitors kept appointments. Will work closely with APP medical staff and Medical Director on missed appointments.
- Assist Providers in the identification of patients based on risk and behaviors for participation in ROM program.
- Monitors and evaluates the progress of the patient regarding compliance with multidisciplinary care plan for minimum of three (3) months.
- Assist patients/providers/triage in completing forms as needed.
New Patients:
- Schedule follow up appointments after new patient visit (go into exam room before patient leaves the clinic).
- Gathers and confirms patient information prior to a patient’s scheduled appointment so clinic physician has all the necessary information in the electronic health record prior to patient appointment, if necessary.
- Assure patient has completed Phreesia prior to appointment; assist patient to complete Phreesia as needed.
- Review patient charts at least 3-5 days before appointment to make sure everything is ready to go for appointment.
- Assist with gathering outside records (referring providers notes, imaging reports, medication lists, past surgical history) that relates to visit with the medical records team.
- Communication with referring and/or primary care physician/provider on scheduled, canceled and/or rescheduled new patient appointment by fax, call or letter adhering to CPIhealth’s policies. Send out letters/calls to new patient and/or Referring & Primary providers per CPIhealth polices.
- Review report of new appointments added each day to ensure charts are prepared prior to patient appointments.
- Ensure expectations and metrics met / exceed expectations in conjunction with clinic goals.
Other Duties:
- Relieves Front Desk and Scheduling lunches (1-2 hours per day).
- Fills in Front Desk/Scheduling when needed.
- Participates in regular team meetings and peer review activities.
- Assist and educates patients as needed with any questions/concerns.
- Aids in clinic as needed.
- Performs other duties as assigned to support the mission, values, and strategies of CPIhealth.
- Demonstrate general knowledge of clinic procedures and answers general questions related to them.

- High School diploma or equivalent required, prefer pre-med or other health-related post graduate fields.
- Previous medical scribe experience preferred but not required.
- Strong knowledge of medical terminology.
- Strong organizational, judgment, communication, and analytical skills; sometimes under pressure.
- Ability to multi-task and perform multiple priorities.
- Cooperative and professional behavior toward patients, peers, providers, management, and visitors.
- Ability to promote favorable facility image with patients, providers, insurance companies and general public.
- Ability to work independently, make decisions, and solve problems.
- Evidence of leadership qualities. Consistently contributing to the improvement of the department and participate in committee participation, task force, etc.
- Ability to contribute to a team environment and provide excellent customer service.
- Working knowledge of Microsoft Office.
Schedule: Full-time, 40 hours per week
