Position Details

Responsibilities
- Conduct skilled physical therapy evaluations and treatment visits in the patient's home
- Complete and submit OASIS assessments accurately and on time
- Develop and implement individualized plan of care in coordination with the physician and care team
- Provide gait training, strengthening, balance, transfer training, fall prevention, and ADL retraining
- Educate patients and caregivers on home exercise programs and safe functional mobility
- Maintain timely documentation in compliance with Medicare and agency standards
- Communicate clinical updates and changes in patient condition to the clinical team
Requirements
- Active California Physical Therapy license in good standing
- Minimum 1 year of experience
- OASIS competency required — must be able to complete independently
- CPR certification current
- Reliable transportation and valid California driver's license
- Strong documentation and communication skills
