Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Clewiston, Florida.

POSITION OVERVIEW
• Community-based RN Case Manager role focused on patient advocacy, care planning, and coordination of complex medical needs
• Opportunity to conduct assessments within home and clinic settings while supporting long-term patient outcomes
• Outpatient nursing environment centered around care coordination and interdisciplinary collaboration
• Manageable caseload allowing focused and relationship-driven patient support
• Role supporting diverse patient populations within community healthcare setting

SCHEDULE

COMPENSATION & BENEFITS
• Base salary: $75,000-$95,000 annually
