Location: Leominster, MA

Schedule: Full-Time (35 Hours Per Week)

Work Environment: Field Based, Hybrid

Compensation: $25-$28 per hour

In this role, you will help protect older adults experiencing abuse, neglect, self-neglect, or financial exploitation. Working directly with consumers, families, healthcare providers, law enforcement, and community agencies, you will play a critical role in helping older adults remain safe while preserving their dignity, independence, and right to self-determination.

What You'll Do

As a Protective Services Case Manager, every day is different. You may:

  • Meet with older adults in their homes to assess concerns related to abuse, neglect, self-neglect, or financial exploitation.
  • Investigate reports and gather information from consumers, family members, healthcare providers, and other involved parties.
  • Develop service plans that help consumers remain safe while respecting their rights and personal choices.
  • Connect consumers to housing resources, healthcare services, legal assistance, behavioral health supports, and community programs.
  • Work collaboratively with hospitals, law enforcement, attorneys, financial institutions, housing agencies, and community organizations.
  • Provide ongoing support and case management for older adults experiencing complex and challenging situations.
  • Advocate for consumers and help them access resources that improve their safety and quality of life.
  • Maintain accurate documentation and case records in accordance with state regulations and agency requirements.
  • Participate in an after-hours on-call rotation to respond to urgent consumer needs.
  • Build strong relationships with community partners to support positive outcomes for consumers.

What We're Looking For

Required Qualifications

  • Bachelor's degree in social work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, Gerontology, or a related field and two years of relevant experience.

OR

  • Master's degree in social work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, Gerontology, or a related field and one year of relevant experience.
  • Experience in counseling, case management, crisis intervention, protective services, healthcare, social services, or related fields. 
  • Experience in Protective Services, Adult Protective Services, crisis intervention, or elder services.
  • Experience working collaboratively with healthcare, legal, and community-based organizations.
  • Strong communication, assessment, and interpersonal skills.
  • Ability to work independently and exercise sound professional judgment.
  • Knowledge of medical, behavioral health, and social service systems.
  • Valid driver's license required.
  • Access to a legally insured and registered motor vehicle.
  • Ability to travel throughout ASNCM's service area and conduct community and home visits.

Preferred Qualifications

  • Bilingual Spanish/English skills are highly desirable.
