The Opportunity
This is a high-impact, high-visibility role at the intersection of clinical workflow, AI, and patient advocacy. As a Senior Product Manager on our product team, you will own the discovery and delivery of transformational features that directly affect the health and safety of millions of seniors transitioning from acute care settings to home each year. You will work closely with the clinical advisors, engineering leads, and our industry’s preeminent payor/health system/post acute partners to translate complex care transition challenges into elegant, evidence-based product experiences. Furthermore, you will be in the frontline of defining the Senior PM role with new tooling — agentic workflows, context-driven spec systems, & AI-assisted discovery. We're investing in our people with cutting edge tools and looking for people excited to operate at the next level. This role isn't for someone whose definition of PM work centers on running ceremonies, curating a roadmap, or translating requirements. You'll own a domain, not a feature list. You'll have a clear path toward director-level responsibilities as you grow in your scope.
What You'll Own
Product Strategy & Vision
- Define and evangelize the roadmap for care transition features — from hospital discharge planning through 30/60/90-day post-acute follow-up with focus on skilled nursing transitions
- Identify white-space opportunities by synthesizing clinical evidence, patient journey research, and market intelligence – and recognize when feature requests are pointing at deeper architectural opportunities
- Partner with product leadership to shape annual product strategy, OKRs, and investment priorities for the post-acute domain
Discovery & Validation
- Lead continuous discovery with clinical end-users - shadow care managers and nurses in real workflows, not just run interviews
- Design and run rapid experiments (prototypes, pilots, A/B tests) to validate problem-solution fit before full build, taking advantage of modern AI-assisted prototyping to move faster than traditional cycles allow
- Translate qualitative insights and quantitative data into well-scoped, high-confidence hypotheses and into concrete specs engineering can act on immediately
Delivery & Execution
- Own the full sprint lifecycle — from story writing and acceptance criteria to UAT sign-off and release, using our AI centric tool sets to spend more time on judgment and less on process
- Collaborate with engineering and design to ship features that are clinically accurate, HIPAA compliant, and delightfully usable
- Maintain a healthy, prioritized backlog that balances strategic initiatives with technical debt and partner commitments
Outcomes & Impact
- Define and track product KPIs tied to clinical outcomes, efficiency gains, and patient experience
- Build feedback loops with customer success and clinical operations to continuously improve post launch
- Report on product performance and key learnings to executive, customer, and clinical leadership
- 5+ years of product management or product ownership experience, with at least 3 years in healthcare technology
- Deep domain knowledge in post-acute care, care transitions, or adjacent areas (home health, SNF, hospice, chronic care management)
- Genuine fluency with — and excitement about — AI-assisted and agentic product workflows, and an interest in helping define what senior PM work looks like as the tooling evolves
- Proven track record shipping complex, regulated software products in Agile/Scrum environments
- Demonstrated ability to work with clinical stakeholders and translate clinical workflows into product requirements
- Strong analytical skills — comfortable with SQL, product analytics tools, and outcome metrics
- Experience navigating HIPAA, CMS regulatory frameworks, and interoperability standards (HL7 FHIR, ADT feeds) preferred
- Experience in digital health, SaaS, or healthcare technology solutions
