Position Overview: This position supports the Non-Clinical Case Management Services contract as a RN Case Manager. This position provides direct non-clinical medical case management services to Vermont Army National Guard (VTARNG) service members, coordinating care across military and civilian medical facilities to promote medical readiness and deployability. The RN Case Manager works under the direction of the Lead RN Case Manager in support of the VTARNG Office of the Deputy State Surgeon.

Location: VTARNG State Medical Detachment, Green Mountain Armory, Camp Johnson, Colchester, VT 05446 (On-site; telework authorized for COOP only)
Anticipated Start Date: 13 July 2026 (Contingent Upon Award)
Position Responsibilities:
- Establish and maintain eCase profiles for all service members referred by a provider; profiles must be created no later than one (1) workday after provider referral.
- Coordinate all aspects of non-clinical case management for VTARNG service members with complex or at-risk medical needs, facilitating care coordination across military and civilian healthcare settings.
- Document all encounters with service members (phone calls, emails, in-person meetings) and newly received medical information in the appropriate system of record (eCase/MEDCHART) within three (3) business days.
- Prepare Soldier records for and attend MEB/PEB boards; review and reconcile hardcopy and electronic case information for file completeness one (1) workday prior to scheduled boards.
- Support and attend scheduled Periodic Health Assessment (PHA) and Soldier Readiness Check/SRP events, including weekend readiness events as required.
- Update and validate MRC3/DRC3 profiles in the eCase module; contribute to the monthly MRC3/DRC3 Profile Management Report.
- Coordinate Soldier treatment plans with appropriate military and civilian medical facilities; communicate with Soldiers’ units regarding scheduled appointments and follow-up documentation.
- Request profile exams, evaluations, and audiology exams within three (3) business days of initial examination.
- Communicate with service members verbally and in writing to confirm understanding of medical documentation submission responsibilities; obtain appropriate release of information and consent forms.
- Ensure all medical documentation is received, scanned, and filed in MEDCHART and the Soldier’s hard copy record; upload medical documents into the Health Readiness Record (HRR).
- Track compliance versus noncompliance with delegated Government personnel during monthly case reviews.
- Maintain HIPAA standards and protect all PII and PHI encountered in the performance of duties.
- Active, unrestricted Registered Nurse (RN) license in good standing; a multi-state Nurse Licensure Compact (NLC) license with Vermont practice privileges is acceptable.
- Minimum of two (2) years of relevant clinical or case management experience.
- Must be able to obtain a Common Access Card (CAC) via a favorably adjudicated Tier 1 (NACI) background investigation.
- Completion of an accredited nursing program.
- Ability to support weekend Soldier Readiness events.
Preferred Skills/Experience:
- Bachelor of Science in Nursing (BSN) or higher.
- Certified Case Manager (CCM) credential or equivalent.
- Previous experience within a Military Health System (MHS) or with military medical readiness systems (MEDCHART, MHS GENESIS, MEDPROS).
