Job Summary
COMHAR is seeking a Behavioral Health Care Coordinator to provide on-site care coordination, behavioral health engagement, and linkage to services for individuals experiencing homelessness. This role supports individuals with moderate to severe behavioral health needs, including those who may be undiagnosed or not currently engaged in care. This position requires a flexible, engagement-focused approach to meet individuals where they are and support stabilization, service connection, and continuity of care.
Key Responsibilities
- Conduct initial and ongoing needs assessments in a shelter-based environment, including behavioral health, medical, and social needs.
- Engage individuals using trauma-informed, person-centered approaches, including those hesitant or not currently connected to care.
- Identify signs and symptoms of moderate to severe mental health and/or substance use needs, including undiagnosed individuals.
- Collaborate with SELF, Inc. shelter case managers to identify needs, coordinate care and align service planning.
- Participate in shelter-based meetings to support coordinated care efforts.
- Facilitate referrals and linkages to COMHAR services including outpatient, medication management, Psychiatric Rehabilitation Services, and supported employment.
- Provide follow-up to ensure engagement in services and address barriers to care.
- Coordinate with medical providers, including COMHAR’s Maria De los Santos
- Clinic, to support individuals with co-occurring health needs.
- Support individuals in crisis through de-escalation and connection to crisis services.
- Maintain accurate and timely documentation in accordance with COMHAR and Self, Inc. standards.
- Support appointment coordination, reminders, and accompaniment as appropriate.
- Adhere to a flexible schedule including evenings or weekends as needed.

Behavioral Health Care Coordinator Job Requirements:
- High school diploma or GED with 12 college credits in behavioral health/human services required.
- Associate's degree or Bachelor's degree (must be in Social Work, Psychology, or other Human Service field) preferred.
- 2–4 years of CMI experience.
- Bilingual proficiency in English and Spanish, with strong reading and writing skills is preferred but not required.
- Current Driver's License; satisfactory driving record; eligible to operate program vehicles.
- Strong engagement skills with individuals experiencing homelessness and complex behavioral health needs.
- Knowledge of trauma-informed care, harm reduction, and recovery-oriented practices.
