Cornerstone Family Healthcare is actively recruiting for a Nutrition Peer Educator to join our growing team in Hawthorne.
RATE OF PAY/SALARY: $18.00 per hour

Description of Duties:
- Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F, HVCS policies and procedures and other applicable regulations
- Assist in the maintenance of food inventory, stocking shelves, packing food pantry bags for clients
- Assist clients schedule and keep their medical, mental health and other service appointments
- Conduct follow up activities, including reminder phone calls and appointment accompaniment
- Assist with making referrals to care management and other services as needed
- Participate in training and program development activities
- Maintain documentation of all program services delivered including referral tracking
- Participate in department and agency meetings and regularly scheduled supervision sessions
- Maintain client charts within program standards
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with Cornerstone policies and procedures and the Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned
Education/Experience Required:
- High School diploma or GED familiarity with social services and nutrition a plus.
- Familiarity of life issues facing the target population(s), including HIV/AIDS, behavioral health, or other chronic illness. Understanding of community level work and the importance of collaborating and coordinating with other organizations is needed. Bilingual (English/Spanish) preferred.
