Description

 Job Summary:

The Clinic Nurse Practitioner (NP) or Physician Assistant (PA) provides comprehensive, patient-centered primary care in collaboration with a supervising or collaborating physician, as applicable by state law. The NP/PA is responsible for assessing, diagnosing, and managing acute and chronic health conditions, promoting preventive care, and delivering evidence-based treatment across the lifespan. This role supports efficient clinic operations while ensuring high-quality outcomes through independent clinical judgment, care coordination, and interdisciplinary collaboration.


Essential Duties and Responsibilities:

  • Provides high-quality, patient-centered care in a rural primary care setting, addressing preventive, acute, and
  • chronic health needs.
  • Conducts comprehensive patient evaluations, including medical histories, physical examinations, and diagnostic
  • assessments.
  • Diagnoses and manages acute and chronic illnesses, including ordering, performing, and interpreting diagnostic
  • tests (e.g., labs, imaging, EKGs).
  • Develops and implements individualized treatment plans, including prescribing medications and therapies in
  • accordance with the scope of practice and regulatory guidelines.
  • Manages a diverse patient population, including unscheduled walk-in visits, by performing medical evaluations,
  • developing treatment plans, and ensuring continuity of care.
  • Provides chronic care management (CCM) services, including ongoing monitoring and management of patients
  • with complex or long-term conditions.
  • Performs in-office procedures within the scope of practice (e.g., suturing, wound care, joint injections, minor
  • procedures).
  • Educates patients and families on diagnoses, treatment plans, medications, preventive care, and self-
  • management strategies.
  • Collaborates with physicians and interdisciplinary team members to coordinate care and ensure optimal patient
  • outcomes.
  • Coordinates referrals to specialists and ensures appropriate follow-up care, including review and management of
  • test results.
  • Supports population health initiatives, including preventive screenings, immunizations, and outreach to high-risk
  • populations.
  • Participates in quality improvement initiatives and clinical protocol development to enhance care delivery.
  • Documents all patient encounters accurately and in a timely fashion in the electronic medical record (EMR).
  • Provides leadership and clinical guidance to nursing and support staff to ensure efficient clinical workflow and
  • patient care delivery.
  • Ensures compliance with clinical guidelines, regulatory standards, and organizational policies.
  • Maintains a safe, clean, and well-equipped clinical environment.
  • May delegate appropriate tasks within scope of practice to Registered Nurses (RNs), Licensed Practical Nurses
  • (LPNs), Certified Medical Assistants (CMAs), and other clinical support staff as needed to ensure safe and
  • efficient patient care delivery.
  • Supports clinic workflow coordination and daily operations to ensure efficient patient care delivery.
  • Provides clinical leadership and mentorship to support staff and assists in maintaining high standards of care.
  • Ensure delegated tasks are completed appropriately and in compliance with policies and scope of practice.
  • Performs other duties as assigned to support clinic and hospital operations.

Requirements

 

Education and/or Experience:

  • For Nurse Practitioner: Graduate of an accredited Nurse Practitioner program (Master’s or Doctoral degree
  • required).
  • For Physician Assistant: Graduate of an accredited Physician Assistant program (Master’s degree required).
  • Minimum of 1–2 years of clinical experience in primary care, rural health, or outpatient settings preferred.
  • Experience in chronic care management, care coordination, or population health strongly preferred.

Certificates and Licenses:

  • Current Basic Life Support (BLS) certification required (ACLS may be preferred).
  • Current DEA registration (or eligibility) required.
  • For Nurse Practitioner: Graduate of an accredited Nurse Practitioner program (Master’s or Doctoral degree
  • required). Current national certification as a Nurse Practitioner (e.g., FNP, ANP).
  • For Physician Assistant: Current, active Physician Assistant (PA-C) license in the State of Illinois.
