The Position

The Patient Document Coordinator is a remote role responsible for managing and triaging all patient-related documentation and record needs. This position serves as a key point of contact for patients and internal teams regarding referrals, labs, imaging, outside medical records, prescription-related documentation, insurance-related documentation, and required forms. The coordinator ensures timely follow-up, accurate documentation, and appropriate routing to clinical or administrative teams to support high-quality patient care and operational efficiency. 


Responsibilities:

Document & Records Triage

  • Serve as a primary contact for incoming calls related to patient documents, records, and paperwork. 
  • Triage and manage lab results, imaging (MRI, CT, OCT, etc.), and outside testing or physician notes. 
  • Identify missing, incomplete, or outdated documentation and follow up with patients, referring offices, or external facilities as needed. 
  • Following up on external referrals and patients appointments. 

Patient Communication & Follow-Up

  • Proactively contact patients regarding missing records, labs, imaging, or documentation required for care. 
  • Answer patient questions related to document status, next steps, and timelines. 
  • Provide clear guidance to patients on how to submit required documentation. 

Forms & Administrative Documentation

  • Manage and process documentation related to: 
  • FMLA and disability paperwork 
  • Workers’ compensation cases 
  • Insurance verification or missing insurance information 
  • Employer, legal, or third-party medical forms 
  • Track form deadlines and ensure timely completion and follow-up. 
  • Communicate status updates to patients and internal teams. 

Coordination & Collaboration

  • Work closely with clinical teams, PSCs, scheduling, prior authorization, and billing teams to ensure documentation supports patient care and scheduling. 
  • Coordinate with external providers, imaging centers, labs, employers, and insurance companies as needed. 
  • Serve as an escalation point for complex or unresolved document-related issues. 

Quality, Compliance & Organization

  • Maintain accurate, timely documentation in the EMR and document management systems. 
  • Ensure compliance with HIPAA and organizational policies related to patient records. 
  • Follow established workflows while identifying opportunities to improve document handling and turnaround times. 
  • Monitor task queues and ensure follow-through on open items. 
  • Other duties as assigned. 
Requirements

Who You Are

  • Experience in healthcare administration, medical records, patient services, or care coordination. 
  • Strong understanding of medical documentation, referrals, and healthcare workflows. 
  • Excellent communication skills and ability to explain processes clearly to patients. 
  • Strong organizational skills with attention to detail. 
  • Ability to manage multiple tasks and deadlines in a remote environment. 

Performance/Skills Requirements:

  • Experience working with EMR systems. 
  • Familiarity with insurance processes, prior authorizations, and workers’ compensation. 
  • Experience handling prescription refill workflows. 
  • Prior experience in a remote or centralized healthcare role. 

Additional Job Details 

  • Fully remote position if you reside in one of the following states: CO, FL, KS, MA, MI, MO, NM, OR, PA, SC, VA, WY, TX or NC. 
  • Compensation for this role will be $18-$24 an hour based on experience.
  • Requires reliable internet access and a secure, private workspace. 
  • May require handling high call volumes and time-sensitive documentation.
