Description
We're looking for a dedicated LVN to provide full-time, one-to-one care in the home setting. You'll be assigned to a single patient, delivering consistent, hands-on care under the supervision of our RN Case Manager. This is a great fit for an LVN who values building a real relationship with their patient and family and wants steady, full-time work.
What You'll Do
- Provide one-to-one bedside care for an assigned patient in the home setting
- Deliver care according to the plan of care established by the RN Case Manager and physician
- Monitor and document the patient's condition, reporting changes to the RN Case Manager
- Support the patient and family with consistency, compassion, and professionalism
- Maintain accurate, timely documentation in line with regulatory and payer requirements
Clinical Responsibilities & Required Skills
- Tracheostomy & ventilator care: Care for trach/vent-dependent patients, including suctioning, trach care, ventilator monitoring, and administration of nebulized medications
- Enteral nutrition & medication: Administer G-tube and J-tube feedings (bolus and continuous) and deliver medications via enteral routes, monitoring tolerance and site care
- Activities of daily living: Provide daily ADLs, including bathing, dressing, grooming, and hygiene
- Mobility & positioning: Perform routine turning and repositioning to maintain skin integrity and prevent pressure injuries
- Ongoing monitoring: Observe respiratory status, nutritional tolerance, and overall condition, escalating concerns to the RN Case Manager as needed

Requirements
- Active, unrestricted LVN license in CA
- Home health or one-to-one care experience
- Experience with trach/vent, G/J tube, and medically complex patients strongly preferred
- Strong clinical skills and the ability to work independently in the home setting
- Compassionate, reliable, and committed to consistent care
- Excellent communication with patients, families, and the RN Case Manager
- Current BLS/CPR certification
- Reliable transportation and willingness to travel to the patient's home
