About the Role:
As our Hospice Licensed Practical Nurse, you will be the cornerstone of support, comfort, and top-tier medical care for patients in their homes or designated facilities. Your Day-to-Day:
- Conduct in-depth patient assessments, diving deep into physical, emotional, and holistic needs in the Greensburg, Waynesburg, and Washington areas.
- Build tailored care plans that prioritize the voice and wishes of our patients and their families.
- Be the crucial link, communicating vital updates between physicians, patients, and caregivers.
- Teach and mentor families, imparting key self-care techniques.
- Lead by example, ensuring a safe and nurturing environment for every patient.
- Take part in our interdisciplinary meetings, driving collaborative patient care.
- Embrace on-call duties, being the pillar of support as defined by the Clinical Manager.
Are You The Perfect Fit?
- Experience: At least 3 years as a Licensed Practical Nurse (LPN), with 1 year in Hospice Care.
- Licensure: Current State Board of Nursing License (LPN) in Pennsylvania & valid CPR certification.
- Skills: Superior organizational & communication abilities.
- Drive: A vehicle, valid driver’s license, and insurance.
