We're looking for a compassionate and motivated Physical Therapist to join our collaborative rehabilitation team. What You'll Do
As a Physical Therapist, you'll evaluate patients and develop individualized treatment plans to restore function, improve mobility, relieve pain, and maximize independence. Working alongside physicians, nursing staff, and the interdisciplinary care team, you'll help patients achieve their rehabilitation goals while delivering exceptional care. Responsibilities include:
- Evaluate patients and develop individualized treatment plans.
- Provide skilled physical therapy interventions.
- Establish realistic and achievable rehabilitation goals.
- Monitor patient progress and modify treatment plans as needed.
- Document patient evaluations, treatments, and progress in accordance with state and federal regulations.
- Educate patients and families on therapy techniques and home exercise programs.
- Collaborate with physicians, nursing staff, and the interdisciplinary care team.
- Provide clinical supervision and guidance to Physical Therapist Assistants (PTAs).
- Participate in care planning and MDS processes.
- Maintain accurate, timely documentation and regulatory compliance.
- Perform other duties as assigned.

Education
- Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy program.
Licensure
- Current Oregon Physical Therapist license.
- Current CPR certification.
Preferred Experience
- Six months of experience in a skilled nursing or long-term care setting preferred.
Knowledge, Skills, and Abilities
- Ability to maintain confidentiality.
- Knowledge of HIPAA regulations.
- General understanding of managed care.
- Knowledge of state and federal regulations related to therapy services and documentation in skilled nursing facilities.
