Case Management Specialist
Department: Tribal Assistance
Exemption Status: Non-Exempt
Pay Grade(s): 8/9 ($27.29 - $30.56 + DOE)
Purpose: The Case Management Specialist will provide case management services, coaching, mentorship, and guidance for General Assistance (GA) and Temporary Assistance to Needy Families (TANF) clients. The incumbent will use a wrap-around service approach to provide professional-level casework and services including, but not limited to: conducting interviews to determine need, assisting client with the application process, collecting data and documents to establish program eligibility, working with the client to develop a strengths-based Case Plan, and monitoring client progress regularly with the end goal of reducing or eliminating the barriers to the client’s achievement of economic independence and monitoring participant progress towards achieving the goal of self-sufficiency as directed under the PL 102-477 plan and program regulations.
Essential Functions. An individual in this role must be able to perform the following functions with or without reasonable accommodation:
· Primary contact in addressing the needs of GA and TANF participants
· Work with participants to identify strengths and develop Case Plans (CP)
· Provide confidential case management services including initiating referrals and follow-up communication with other service providers, complete home visits, and attend appointments with participants as necessary in an advocate capacity, etc.
· Responsible for reading, understanding, and explaining State, Federal, and Tribal policies related to participant services for GA and TANF programs.
· Engage appropriately with returning participants to support them in identifying strengths, existing supports, and community resources
· Work with each participant on their financial wellness and create a financial plan.
· Interview participants and make recommendations and referrals
· Explain program options and services available to the participant, and the requirements of each program
· Ensure participants’ right to privacy and ensure appropriate confidentiality at all times including notifying supervisor if there is a conflict of interest.
· Assist clients in completing necessary paperwork, follow-up, and assist participant in acquiring necessary documents via a release of information
· Responsible for daily case-noting all contact with participants and service providers using a designated database
· Work with participants in setting goals/objectives or identifying barriers to self-sufficiency or obtain permanent housing
· Process internal paperwork for the participant in a timely manner to ensure program goals are met
· Work with employers to ensure participants are meeting their employment responsibilities
· Conduct interviews to determine needs
· Work within timeframes and deadlines dictated by program requirements
· Conduct self-audits of participant files to correct reporting or data entry errors and make adjustments if necessary.
· Investigate complaints from participants and others to verify validity; detect discrepancies, and seek clarification and verification of data.
· Work with other agencies including; housing, employment, local employers, and other service providers along with internal providers of services and departments to provide holistic wrap-around services to participants.

· Experience working with Native American/Alaska Native clientele; sensitive to the cultural diversity and needs of this population
Minimum Qualifications (education, experience, skills):
· Associate degree
· Two (2) years' experience working in the social work field or a closely related position.
· Must be 21 years of age and have a valid driver's license.
· Substitution: High school diploma and two (2) years' experience in social services or employment services or closely related position.
Preferred Qualifications (education, experience, skills):
· BA degree in Social Services or related field
· LBSW – Licensed by the State of Alaska
· Accredited Case Manager
· CPR/ First Aid/ NARCAN trained or willingness to be certified soon after date of hire

· The majority of work is performed in a professional office setting and is generally sedentary, requiring routine walking, standing, bending, and carrying of items weighing less than 40 lbs.

· Travel on small aircraft or ferry may be required.
· Certain positions at Tlingit & Haida may have exposure to certain hazardous materials or conditions. The supervisor will review these if applicable.
