L'Acadie in Van Buren has an exciting opportunity for a CNA to join their team. The successful candidate will provide direct and indirect patient care duties under the guidance and direction of the charge nurse. Responsible for carrying out components of the plan of care relating to basic human, social and emotional needs; comfort, safety and emergency measures; activities of daily living, personal care needs, nutrition and fluid needs, and elimination needs. Reports any incidents or abnormal findings to the charge nurse. Exhibits teamwork skills. Promotes patient advocacy. Supports the organization's vision and mission. Utilizes knowledge of patient's age and culture in provision of patient care.


Essential Job Duties: 

Demonstrates Competency in the Following Area : 


Age Appropriateness: 

Promotes dignity and respects the rights and choices of the individuals residing at L' Acadie. Demonstrates a knowledge of the following as it relates to the residents they serve: Growth developmental factors; Risk factors/preventative health measures; Developmental age as it relates to Activities and Programming. Documents concerns and issues related to the age of the individual. Everyone is treated as an individual first. Assists individuals to care for personal property (i.e. clothes, room, CD's, tapes). Promotes L' Acadie as the individual's home. Reports any concerns or issues to the charge nurse in a timely manner. 

Performs/assists with fundamental nursing care, providing for patient's personal, safety, and comfort needs. 


ADL's: 

Provides hygienic care, including bed bath/shower, hair care, shave, oral care, bed linen change based on the active treatment plans of residents. Be familiar with adaptive equipment such as hearing aids, braces, wheelchairs, etc. Performs HS care as a component of evening shift responsibilities. Must immediately report any abrasions, cuts, bruises, falls, accidents, behaviors to the charge nurse. Must immediately report any indications of potential abuse or neglect to the charge nurse. 


Continuous Active Treatment: 

Participates in recreation and leisure time activities with residents and ensures active treatment plans are followed timely and accurately. Is able to implement all ancillary services (OT/PT/Speech and others). Familiar with the Goals/Objectives as designed by the Person Centered Planning Process and Nursing Plan of Care Process. Participates in meal preparation, understanding each individual's needs. Documents progress accurately and in a timely fashion on data sheets provided for each resident. Assures familiarity with new active treatment plans within 7 days of implementation. Is able to handle behavioral incidents appropriately. Reports any incidents to the charge nurse as soon as possible. Accurately and objectively documents incidents on the appropriate forms. Always assists the individuals we serve to attain the highest level of independence possible. Assures that Active Treatment is continuous. Must be able to and ready to give input regarding a resident's progress during the Person Centered Plan process. May be responsible for additional tasks and assignments as indicated by the charge nurse or manager. Is responsible to follow and implement each resident's Nursing Plan of Care and Person Centered Plan as outlined. Is responsible to accurately document each resident's response to individual active treatment plans. 


 Transfer/ Ambulation/Safety: 

Assesses the need for and utilizes patient transfer and friction reducing devices, mechanical lift, and appropriate number of assistants to safely reposition or transfer patients, following the no manual lift policy and proper body mechanics. Ambulates patients as directed by nursing. Assures patient safety by leaving bed in low, locked position, wheelchair brakes applied. Utilizes TABS and other safety measures correctly as outlined in resident's PCP and safety plan. 


Infection Control: 

Performs general care activities for patients with special precautions ( contact, droplet, or airborne), utilizing appropriate technique and following infection control policies. Performs proper hand washing hygiene before and after client care. 

Communicates effectively and performs in harmony with visitors, coworkers. physicians. Demonstrates ability to establish a therapeutic relationship with the resident, guardian, family and visitors through communication. Approaches interpersonal relationships in a manner which avoids antagonism, reduces conflict, and prevents undue resident anxiety. Recognizes the need for team effort by cooperating with rotation of assignments and assisting other members of the healthcare team. 


Demonstrates responsibility and accountability: 

Assumes personal and professional responsibility for decisions made and actions taken. Assumes personal and professional responsibility for maintaining requirements and for following policy. Maintains a current CNA certificate from the state of Maine. Follows through with requests from nurse manager/supervisor in a timely manner without multiple reminders. 


Other Duties:

Complies with local, state and federal laws and regulations.

Requirements
  • Meets a minimum of 12 hours per year of continuing education (hours submitted to the Health Education Department for recording).
  • Meets the unit’s annual competency training requirements.
  • Maintains current Heartsaver or BLS.
  • High school diploma or G.E.D. preferred.
  • CNA/RA-Certified Nurses’ Assistant certification.
  • DTA – Certified Nurses’ Assistant certification, 60 hours in-service training 3 months full time experience working with individuals with developmental disabilities - if applicable


Schedule:

  • With consideration to our patients’ and staff needs, you may need to work on days, evenings, nights, weekends and/or holidays and may need to float to other units.
