Remote Care Coordinator (Medical Assistant) 


Part-Time | 8-29 Hours Weekly | Spanish Preferred


At TimeDoc Health, we're on a mission to help patients with chronic conditions stay healthier and more connected to their care. We're looking for compassionate, bilingual Medical Assistants who enjoy building relationships, supporting patients, and making a meaningful impact every day.


What You'll Do

  • Conduct monthly Chronic Care Management (CCM) calls with patients
  • Support patients with chronic conditions between provider visits
  • Educate patients on their health conditions and care plans
  • Coordinate care with providers, pharmacies, caregivers, and community resources
  • Document patient interactions, update care plans, identify gaps in care, and connect patients with available resources
  • Spend the majority of your shift making and receiving patient calls while providing personalized support and care coordination


Qualifications

  • Active Certified or Registered Medical Assistant credential required
  • Fluent in English and Spanish preferred (verbal and written communication)
  • Minimum 1 year of Medical Assistant experience
  • Strong communication, documentation, and critical thinking skills
  • Experience with EMR/EHR systems, Microsoft Office, and Google Workspace preferred
  • Care coordination, case management, call center, triage, or geriatric experience is a plus


Schedule Requirements

  • 8-29 hours per week
  • Preferred schedule between 10:00 AM - 6:00 PM ET
  •  Flexible 4-hour shift options available:
    • 10:00 AM - 2:00 PM ET
    • 2:00 PM - 6:00 PM ET
  • One weekend per month required
  • Must be available to work at least 2 holidays per year


Remote Work Requirements

  • Private home office free from distractions
  • Company laptop and software provided
  • Second monitor required
  • Reliable internet with upload speeds of at least 100 Mbps
  • Satellite internet and mobile hotspots are not compatible with this role
