Progressive Directions, Inc. is dedicated to supporting individuals with intellectual and developmental disabilities through quality healthcare, advocacy, and person-centered services. We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our nursing team.
Position Summary
The Licensed Practical Nurse (LPN) provides nursing care and healthcare oversight for individuals receiving services in residential and community settings. This position works closely with the Agency RN, physicians, therapists, direct support professionals, and families to ensure the health, safety, and well-being of individuals served.
Essential Responsibilities
- Administer medications and monitor for effectiveness and adverse reactions.
- Implement and monitor Individualized Health Care Plans and Medical Crisis Plans.
- Provide nursing support including medication administration, routine treatments, health monitoring, and documentation.
- Accompany individuals to medical appointments and coordinate follow-up care.
- Maintain accurate medical records, MARs, Health Passports, medication counts, and healthcare documentation.
- Monitor health concerns including seizures, falls, weight changes, dietary needs, bowel and bladder function, and chronic medical conditions.
- Ensure healthcare supplies and medical equipment are properly maintained.
- Assist with training and mentoring new staff regarding healthcare procedures and individual support plans.
- Participate in ISP meetings and collaborate with interdisciplinary teams.
- Ensure compliance with DDA, Department of Health, agency policies, and nursing standards.

Requirements
Qualifications
- Current Tennessee LPN license in good standing or Compact State License accepted by Tennessee.
- Valid driver's license with acceptable driving record.
- Current BLS (or ability to obtain).
- Ability to successfully complete DDA-required training.
- Strong assessment, documentation, communication, and organizational skills.
- Experience supporting individuals with intellectual and developmental disabilities preferred.

Physical Requirements
- Ability to stand, walk, bend, stoop, and assist individuals as needed.
- Ability to lift and move up to 50 pounds.

Salary Description
30.00 per hour
