About the Position:

You will help clients reintegrate into their daily lives by offering therapeutic follow-up, mentorship, and ensuring a high standard of service. You'll manage client scheduling, facilitate program activities, and provide individual case management to support recovery in sober living. To provide the necessary guidance, support, and therapeutic follow up that will enable each client to effectively utilize tools and resources throughout their time in Extended Care and as they reintegrate into their daily lives. The expected duration of this position is approximately 3-6 months.


Schedule:

Monday - Friday: 8:00 am - 4:30 pm


Your Role:

  • Provide ongoing support, follow-up, and case management to Extended Care clients, ensuring a safe, structured, and recovery-focused environment.
  • Build and maintain effective communication with clients, families, referral sources, and interdisciplinary team members.
  • Support admissions and onboarding, including preparing rooms, completing intake documentation, greeting clients and families, and orienting new residents to program expectations, services, and house guidelines.
  • Monitor client progress, behavior, attendance, and adherence to house expectations; promptly communicate concerns, behavioral issues, or safety risks to appropriate clinical staff and leadership.
  • Conduct routine room inspections and report property issues, maintenance needs, or guideline infractions.
  • Provide mentorship, accountability, and real-time guidance to residents to support recovery goals and successful community living.
  • Assist clients with recovery planning, including sponsorship opportunities, transportation coordination, recovery support meeting attendance, and access to community resources.
  • Facilitate groups, educational programming, and recovery-focused activities as assigned.
  • Coordinate care with internal and external providers, including outpatient services, medical providers, families, and referral sources to support continuity of care.
  • Conduct individual, group, and family support sessions as appropriate to assist clients in recovery and treatment progress.
  • Provide weekly case management sessions and progress updates to designated contacts, ensuring timely and accurate documentation.
  • Maintain accurate client records, reports, case histories, attendance, and service documentation in designated systems, including One Step and Aura.
  • Respond appropriately during crises and provide support to ensure client safety and continuity of care.
  • Support overall program operations and collaborate with team members to deliver exceptional client care and service.

Qualifications:

  • High School Diploma or equivalent required
  • Current or working towards certification (CAC, CASAC, CPRS) preferred
  • Minimum 1 years’ experience/knowledge in substance abuse treatment preferred
  • Clean Driving Record required

Compensation:


The base rate of pay for this position is $21.00 to 25.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.

Salary Description
$21 - $25
