Description
This position is within a positive and recovery focused agency that really improves lives. The RN works directly with members to provide prescribed physical health medical services that improve chronic health conditions and annual wellness exams for members who also have mental illness and/or substance abuse conditions. Responsible for regular interactions with our membership to gather medical information and data as directed by Nurse Practitioner and sharing necessary labs, results or concerns for coordination of care with primary clinical team and medical specialists. Document all services to provide clarity and collaboration with other health care professionals. Ensures proper illness and injury care and disease prevention, treatment, and recovery. There is also some limited health benefits to our employees that may be provided. Maintains exam room and pharmacy supplies in accordance to medical best practices and licensing requirements. Coordination of care with other providers and teaching healthy living skills are significant aspects of this position.
Requirements
Essential Functions:
In conjunction with the Nurse Practitioner, assures that the delivery of nursing care and documentation meets licensing regulations and accreditation standards.
Assures the availability of medical and nursing supplies and equipment. Monitors the safety and effectiveness of equipment in accordance with licensing rules, STAR Clinic Policies and Procedures, AHCCCS and other contractual requirements.
Oversees the inventory of STAR pharmacy supplies and medications, as well as properly purging expired or damaged items with proper documentation, per STAR policy and procedures.
Completes and/or reviews of all required reports such as:
Incident reports
Infection control reports
Medication errors
Adverse drug reactions
Safety Intervention documentation
Develops and facilitates individual and group classes on healthy living, tobacco cessation, living well with Diabetes and other health conditions. Proper documentation of class participates within the medical record.
Performs specimen collection for blood, urine, etc. Administers injections or other medical procedures within the directive of our nurse practitioner.
Ensures completion of designated assessments of all members brought to the facility developing plans and recovery solutions in partnership with the member.
Travels weekly to our other two locations to run 2 groups a day and perform health assessments with 4 members as directed by prescriber.
Documents information and service provided in electronic data base assuring continuity of the service record and as a tool for communication with other team members.
Provides appropriate treatment and makes appropriate dispositions and referrals in consultation with the team.
Provides health care including all required nursing treatments and member monitoring, as noted in STAR policies and procedures.
Administers medication following STAR Clinic policies and procedures.
Initiates and/or ensures appropriate response to all medical emergencies in accordance with agency medical protocols and agency procedures.
Oversees safety intervention procedures as appropriate to program.
Communicates complaints and incidents to supervisory personnel in completing require documentation
Performs staff TB skin tests, Drug Screens, other labs per directive of NP and approved by CEO/HR.
Communicates with supervisor and site managers if there are changes in the RN’s schedule which includes time off or emergency absences.
Education
Bachelor’s Degree with RN license Required
Experience:
1 or more years of work experience Required
Knowledge, Skills & Abilities:
Strong computer skills
Knowledge of public behavioral health programs and services
Excellent time management, organizational and communication skills
Must be professional, display leadership qualities
Work independently, and be a team player
Proven clinical and communication skills.
Basic knowledge of proper codes and billing for services rendered.
Professional Certifications & Licenses:
Graduation from an accredited School of Nursing as a Registered Nurse. Required
Valid AZ State RN license or Multistate Preferred
Other requirements:
Self-identify as a Peer by the InterNational Association of Peer Supporters (iNAPS)
or Mental Health America definition and/or have (or have had a history) of a
Mental Health/Behavioral Health or Substance Use/Abuse Diagnosis
