Description

The Wellness Monitor provides peer support services by helping maintain a safe, supportive and recovery-oriented environment for members, staff, and visitors. Through regular engagement and presence throughout the facility, this role supports member well-being, encourages recovery goals, and fosters a positive community atmosphere. The Wellness Monitor provides assistance and support during all hours of operations, promotes wellness and community standards, responds to concerns as they arise, and contributes to the overall well-being of the outpatient clinic. 


Responsibilities:

The following duties are only intended to provide a general outline of the duties of an Outpatient Program Assistant at Axiom Care.


· Provide peer support to members by promoting recovery, wellness, and positive engagement throughout the clinic environment

· Maintain a consistent and supportive presence throughout the facility and surrounding grounds to foster a safe, welcoming and recovery-oriented atmosphere

· Conduct regular rounds of the facility and grounds to observe members’ needs, identify concerns and offer support and assistance when appropriate

· Engage members in respectful, recovery-focused conversations and encourage adherence to clinic guidelines and community standards

· Redirect members who are not following program rules, behavioral expectations, or safety guidelines using trauma-informed, person-centered communication techniques

· Assist in de-escalating conflicts by addressing disruptive behaviors while maintaining dignity and respect for all individuals 

· Communicate member concerns, behavioral issues, safety risks, and facility needs to appropriate staff in a timely manner

· Document all interactions, observations and interventions accurately, consistently, and in accordance with clinic policies and confidentiality standards. 

· Complete required incident reports and documentation within established timeframes.

· Participate in team meetings, trainings, and other activities that support quality care and member recovery. 

· Perform other duties as assigned by supervisor

Requirements

Required Skills:

  

· Knowledge of Windows, basic email, Microsoft Word, timekeeping software, and basic computer navigational skills.

· Strong organizational skills

· Ability to multi-task

· Ability to treat confidential information with appropriate discretion

· Ability to use general office equipment (scanners, printers, fax machines, etc.)

· Ability to write professional correspondence and reports free from grammatical and spelling mistakes.

· Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

· Shift Flexibility


Education and Experience:


· High school diploma or equivalent

· Peer Support Certificate
