Key Responsibilities:

- Conduct comprehensive assessments to identify clients' needs, strengths, and risk factors

- Develop individualized service plans in collaboration with clients, setting achievable goals

- Provide ongoing case management, including counseling, resource linkage, and advocacy

- Monitor client progress and adjust service plans as necessary to ensure positive outcomes

- Facilitate access to community resources, support networks, and preventive programs

- Maintain accurate and timely documentation of client interactions and service delivery

- Collaborate with multidisciplinary teams, including social workers, healthcare providers, and community organizations

- Promote client engagement and retention through empathetic communication and support

- Ability to create rapport and engagement with youths.

Requirements

 

Skills and Qualifications:

- Bachelor's degree in social work, psychology, human services, or a related field (Master’s preferred)

- Proven experience in case management, social services, or prevention programs

- Strong interpersonal and communication skills, with the ability to build rapport with diverse populations

- Knowledge of community resources, social service systems, and prevention strategies

- Ability to assess client needs and develop effective intervention plans

- Excellent organizational skills and attention to detail

- Ability to work independently and as part of a team in a fast-paced environment

- Valid driver’s license and reliable transportation may be required

- Bilingual (English/Spanish)
