Licensed Practical Nurse (LPN)/ Resident Care Coordinator (RCC)
Description

We are seeking an experienced and dependable LPN / Resident Care Coordinator (RCC) to join our clinical leadership team. This role is hands-on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed. This is a working nurse role, not a desk-only position.


Key Responsibilities

  • Provide direct nursing care to residents in Assisted Living and/or Memory Care
     
  • Administer medications and treatments in accordance with physician orders and state regulations
     
  • Serve as RCC by coordinating resident care plans, assessments, and clinical documentation
     
  • Supervise and support Caregivers/QMAPs during assigned shifts
     
  • Assist on the floor as needed with resident care and staffing support
     
  • Communicate effectively with families, providers, and internal leadership
     
  • Ensure compliance with Colorado Department of Public Health and Environment (CDPHE) regulations
     
  • Participate in audits, surveys, and quality assurance processes
     
  • Respond to clinical concerns, incidents, and emergencies appropriately

     

Schedule

  • Sunday through Thursday
  • Tuesday through Saturday

Requirements

 Qualifications

  • Current Colorado LPN license in good standing (required)  
  • Experience in Assisted Living, Memory Care, or Long-Term Care preferred  
  • Strong medication management and clinical judgment skills  
  • Ability and willingness to assist with hands-on resident care  
  • Experience supervising or leading care staff strongly preferred  
  • Strong documentation and communication skills  
  • Ability to remain calm, organized, and decisive in a fast-paced environment
Salary Description
$70,000.00
