JOB SUMMARY: This position will provide Prevention navigation services, including but not limited to HIV/STI
testing, pre- and post-test counseling, condom education and distribution, and referrals and/or linkage to prevention and essential services for high-risk HIV-negative persons and persons living with HIV. This position has a mandatory outreach requirement. Staff are required to participate in off-site community outreach activities, including evenings and weekends as needed, and must
provide HIV testing, pre-and post-test counseling, and linkage to rapid entry HIV medical care services during outreach events in accordance with PIHC and funder requirements. This position will provide rapid entry into HIV care services at PIHC, off-site, and through self-based testing in confidential, one-on-one sessions with clients while maintaining the pre-counseling protocol of
no more than 30 minutes per session. This position will fully support the PIHC PrEP program by providing PrEP and PEP education; linkage to same-day services; retention and re-engagement to PrEP care; and administrative support. Staff will utilize established Rapid Entry and Reengagement in Care protocols to link clients to prevention and essential services. This position will track linkage or re-engagement for up to 90 days to ensure clients access their first medical appointment; document all linkage and navigation services; and notify the local health department of all HIV-positive clients for Partner Services. This position reports directly to Prevention Program Manager I and supports targeted Prevention program objectives. This position requires fluency in Spanish and English to effectively serve PIHC’s diverse client population. The Bilingual Prevention Navigator will provide culturally and linguistically appropriate services, including interpretation, translation of prevention education materials, direct client navigation in Spanish to reduce barriers to HIV/ST testing, PrEP & PEP access, and rapid entry to care.

ESSENTIAL FUNCTIONS:
Duties and Responsibilities
- Provide direct HIV/STI testing, counseling, PrEP education, and linkage services in Spanish, and English to ensure equitable access to prevention and care services.
- Translate and/ or review prevention materials, consent forms, outreach messaging, and educational resources to ensure linguistic accuracy and cultural responsiveness for Spanish- speaking communities.
- Serve as a cultural liaison to support engagement, retention, and trust-building within Spanish- speaking populations disproportionately impacted by HIV and STIs.
- Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling Standards
- Conducts offsite testing in a sex positive manner in non-traditional settings during non traditional work hours
- Provide effective condom education. Distribute condoms to target HIV-infected persons and persons at highest risk of acquiring HIV
- Provides education on pre-exposure prophylaxis (PrEP) and ensures persons eligible for PrEP if interested are linked to PrEP.
- Be able to answer general questions about PrEP for both agency staff and for clients, attend community events to promote PrEP, and assist with PrEP marketing strategies.
- Provide PrEP navigation and linkage including screening for eligibility, completing all required documentation and paperwork, and collecting all required fees.
- Coordinate enrollment in PIHC’s Rapid Entry Program for the following patients: newly diagnosed with HIV, living with HIV but has never been placed into care, or not currently engaged to care.
- Assess potential barriers to PrEP care and the impact that stigma has on accessing PrEP.
- Provides education about the importance of medication adherence for persons receiving PrEP medical care.
- Provides referrals to medication engagement services (ME) about the importance of HIV medical care and medication adherence to persons living with HIV.
- Collaborate/consult with PIHC clinic staff, medical case managers, and behavioral health staff for care coordination.
- Schedule medial appointments (labs, clinic visits) for patients with the providers and/or nurses, utilizing Electronic Medical Record (EMR) system.
- Create and update client information into electronic medical system
- Conduct routine follows calls with PrEP patients and patients receiving HIV medical care.
- Attend required Prevention navigation training regularly, cultural competency training annually and other applicable agency trainings.
- Abide by all federal, state, local, and agency policies regarding confidentiality of patient information
- Follows all agency guidelines relating to HIV testing, PrEP protocols, rapid entry services, incentive distribution, and documentation of all program data.

Knowledge, Skills, and Abilities
- Strong written and verbal communication skills, including spelling, grammar, listening, and public speaking.
- Excellent organizational and time management skills.
- Demonstrated fluency in Spanish and English (verbal and written) with the ability to deliver counseling, education and navigation services in both languages in a culturally responsive and trauma-informed manner.
- Ability to work both independently and collaboratively within a team setting.
- Effective problem-solving and decision-making capabilities.
- Ability to manage multiple tasks and meet deadlines in a timely manner.
- Strong project follow-through and accountability for task completion.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).

Minimum Qualifications
- Bachelor’s degree from an accredited institution, in public health, counseling, psychology, or related field
- Fluent in writing/speaking both Spanish and English

- Two years of experience working in the field of HIV, PrEP, or public health; or
- Any equivalent combination of proven training and experience which provides the required knowledge, skills, and abilities to perform this job.

TRAVEL:
• Local travel between PIHC sites and to and from community agencies will be required.
• Occasional travel to events for training and promotion of salient services to AIDS Service
Organizations. Occasional evening and weekend work is required and working greater
than 40 hours per week may be required
