Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Geriatric Case Manager in Naples, Florida.

POSITION OVERVIEW
• Provide short- and long-term case management services for older adults living in the community
• Develop individualized care plans that promote independence and quality of life
• Conduct home visits and ongoing client assessments
• Coordinate community resources and communicate with families and healthcare providers
• Join a mission-driven organization dedicated to helping seniors age safely and independently

COMPENSATION
• Salary: $50,000-$55,000

SCHEDULE
• Monday-Friday
• 9:00 AM-5:00 PM
• No Weekends
• No On-Call Responsibilities

• Bachelor's or Master's Degree in Social Work REQUIRED
• Case Management or Related Experience REQUIRED
• Experience Working with Older Adults Preferred
