The Medical Records Specialist (LPN) is responsible for maintaining accurate and complete resident medical records in accordance with federal, state, and local regulations, as well as company policies and procedures. This position plays a vital role in supporting clinical operations by ensuring documentation is timely, compliant, and readily available while collaborating with the interdisciplinary team to promote quality resident care.
Medical Records Specialist duties include but are not limited to:
- Maintain resident medical records in accordance with company policies and regulatory requirements
- Compile, organize, and update resident medical records in both electronic and paper formats
- Monitor and audit medical records for completeness, accuracy, and regulatory compliance
- Provide daily clinical stand-up reports, including past due documentation, physician orders, admissions, discharges, and clinical alerts
- Coordinate medical record documentation with nursing staff, physicians, hospitals, and other healthcare providers
- Process admission, transfer, and discharge records while completing required audits
- Maintain archived medical records in accordance with company retention policies
- Provide administrative support to the Nursing Department through audits, education, and documentation assistance
- Perform daily, weekly, monthly, and quarterly medical record audits
- Audit skilled nursing documentation and changes in resident condition
- Participate in PDPM Committee meetings and quality improvement initiatives
- Prepare reports and documentation for physicians, facility leadership, company representatives, and regulatory agencies
- Resolve incomplete or conflicting documentation by communicating with physicians and clinical staff
- Prepare emergency hard-copy medical records for disaster preparedness and power outages
- Compile census, demographic, and clinical data as requested
- Oversee documentation compliance within the electronic medical record system and provide education as needed
- Update CNA task assignments based on physician orders and clinical meeting discussions
- Trigger required documentation based on the MDS schedule and facility guidelines
- Serve as the facility's HIPAA Privacy Officer in collaboration with the Administrator and Health Information Management team
- Assist with Quality Improvement (QI) and Infection Control (IC) responsibilities as assigned
- Travel to hospitals, physician offices, or other healthcare facilities when documentation requiring signatures or records retrieval is necessary

- Active Licensed Practical Nurse (LPN) license required
- Previous long-term care experience preferred
- Strong knowledge of medical record documentation, HIPAA regulations, and healthcare compliance
- Excellent organizational, communication, and time management skills
- Ability to work independently while collaborating effectively with the interdisciplinary team
