Position Summary

The Speech-Language Pathologist evaluates, develops, and implements individualized treatment plans for patients with speech, language, cognitive, voice, and swallowing disorders. Working closely with physicians, nurses, therapists, and caregivers, the SLP helps patients maximize independence and improve their quality of life in the home setting.

Responsibilities

  • Perform comprehensive speech and swallowing evaluations.
  • Develop individualized treatment plans.
  • Provide therapy for speech, language, cognitive, voice, and swallowing disorders.
  • Educate patients and caregivers.
  • Recommend adaptive communication strategies.
  • Collaborate with physicians and the interdisciplinary care team.
  • Complete accurate and timely electronic documentation.
  • Participate in case conferences and quality improvement activities.

Qualifications

Required

  • Master's Degree in Speech-Language Pathology
  • Current Virginia SLP License
  • Certificate of Clinical Competence (CCC-SLP) preferred or eligibility
  • Current CPR/BLS Certification
  • Valid Driver's License
  • Reliable Transportation
  • Current Auto Insurance

Preferred

  • Home Health experience
  • Medicare/Medicaid experience
  • OASIS familiarity
  • EMR experience (Alora preferred)
