Advanced Home Health and Hospice of Las Vegas is hiring a full-time LPN (Home Health) to provide skilled nursing care to patients in their homes throughout the Southwest, Northwest, and North Las Vegas.

Schedule

• Full-time
• Monday–Friday
• Days
• 8 Hour Shifts
• Field-based driving required across SW/NW/North Las Vegas (Primarily West Side of Las Vegas)

Responsibilities

• Provide skilled nursing visits in the home under the plan of care
• Perform assessments, vitals, documentation, and patient/caregiver education
• Coordinate with RN case managers and the interdisciplinary team
• Communicate changes in patient condition and follow clinical protocols
• Maintain accurate and timely charting per agency and regulatory standards

• Active Nevada (NV) LPN/LVN License (required)
• Minimum 2 years of home health experience (required)
• Valid driver’s license and reliable transportation (driving required)
