Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Bilingual Registered Nurse (RN) for Element Care PACE in Lynn, Massachusetts.

POSITION OVERVIEW

• Provide comprehensive nursing care for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).
• Perform initial and ongoing nursing assessments in both clinic and community settings.
• Develop, implement, and evaluate individualized care plans that promote independence and reduce hospitalizations.
• Collaborate with physicians, therapists, social workers, and the interdisciplinary care team to coordinate participant care.
• Educate participants and caregivers regarding disease management, medications, and preventative health.
• Maintain accurate clinical documentation within the electronic medical record.

COMPENSATION & BENEFITS

• Competitive salary from $80,000-$104,000 based on experience.

MINIMUM REQUIREMENTS

• Active Registered Nurse (RN) license in Massachusetts.
• Bilingual in English and Spanish.
• Minimum two years of recent Registered Nurse experience.
• Ability to provide nursing care in both clinic and home/community settings.
• Strong assessment, communication, and documentation skills.
• Experience in primary care, geriatrics, home health, or community health is preferred.
• Valid driver's license and reliable transportation.
