Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Physician for Element Care PACE in Lynn, Massachusetts.

POSITION OVERVIEW

  • Provide comprehensive primary care for medically complex older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).
  • Manage acute illnesses, chronic diseases, preventive care, medication management, and annual wellness evaluations.
  • Conduct visits in the PACE center, participant homes, skilled nursing facilities, and through telehealth when appropriate.
  • Serve as the physician leader within an interdisciplinary care team alongside Nurse Practitioners, nurses, therapists, social workers, pharmacists, and care coordinators.
  • Complete hospital follow-up visits and coordinate care transitions to reduce avoidable admissions.
  • Support participant-centered goals that allow seniors to remain safely living within the community.

  • Competitive salary from $200,000-$294,000 based on experience.

Requirements

MINIMUM REQUIREMENTS

  • Board Certified or Board Eligible in Internal Medicine or Family Medicine.
  • Eligible for unrestricted Massachusetts medical license.
  • Active DEA registration or ability to obtain.
  • Experience or strong interest in geriatrics, primary care, or complex chronic disease management.
  • Excellent communication and interdisciplinary collaboration skills.
  • Commitment to patient-centered, community-based senior care.
