LIFE Pittsburgh is a community-based alternative to nursing home care and assisted living. Through the PACE model, we help frail older adults remain safely and meaningfully in their homes by coordinating care across Day Health Centers, in-home support, transportation, meals, therapies, and medical services.
Job Summary: In collaboration with the Management Team, is responsible for the day-to- day operations, administration, coordination of all care delivered to LIFE Pittsburgh Participants by Personal Care Staff. Responsible for and recommends annual budget for center operations and monitors budgets by service and per member per month.
Essential Functions:
- Demonstrates and reflects a commitment to the mission and core values of LIFE Pittsburgh to support the elderly in maintaining their dignity and independence in the community by modeling behaviors that focusses on the need of participants and families, an in-depth knowledge and understanding of allocations and how resources are being utilized and an active advocacy for all PCAs and PCA related areas.
- By demonstrating an in depth knowledge of the content of all PCA onboarding, orientation and training aspects and by demonstrating an working knowledge of existing home care and other related policies, will Collaborate in the development, update and maintenance of PCA Home Care Service Department policies and procedures and ensures that all are being followed.
- By demonstrating an understanding of scheduling challenges, actively helping to resolve staffing shortages, identifying inefficiencies and verbalizing staffing needs, will ensure that PCAs are scheduled productively and effectively. By positively working through conflict and the ability to maintain professional interactions, will demonstrates the ability to work collaboratively with the Community Care Supervisors, PCA Scheduling Associates and Scheduling Supervisor to ensure services are provided as allocated by the team.
- Demonstrates a good understanding of FTE staffing budget, needs and can explain overtime variances. Ensures that proper and effective staffing pattern exists and communicates changes and needs timely and clearly to HR and Director of Home Care Services. (evidence of knowledge of current staffing patterns and staff roster, analysis of home care department reports, can verbalize easily the needs for hire, hours and shift, and why overtime exists)
- Develops, coordinates, implements and monitors the onboarding and orientation of new department staff (including agency staff) by demonstrating an understanding of what, when, who, why and how new staff are orientated. Ensure that Competency program and orientation material/program is based on professional standards of clinical care, regulatory guidelines and program specific needs while working in close coordination with Training and Education Department, managers and administrative team for input, guidance and monitoring as demonstrated by being a clear communicator of this information, collaboration with T/E and other peer manager, being a confident educator/ trainer of staff.
- Participates in the development and implementation of home care staff curriculum to ensure all skills, knowledge and clinical applications are capture and sufficiently trained on and enforced. Ensures clinical and medical reasoning, rationale and consideration are represented in all trainings. Will show evidence of an in-depth knowledge of PCA protocols and techniques as well as can clearly interpret and explain to others the rationale for protocol/ procedures and policies. Shows active participation in content discussions as well as being an active and effective trainer of all area of the home care department. - By demonstrating an active oversight of grievance completion by CCS, directing CCS actions to ensure trends are identified, and long-term fixes are put in place as well as staff retraining, and discipline done as an appropriate and timely part of the process, will participate in the quality program collaboratively to measure operational quality, participant satisfaction and department performance on established protocols and policy in place. Completes or facilitated the accurate and timely completion of grievances
- As evidence of team attendance and care plan attendance, facilitation of meetings about allocations and difficult situations, support the CCS in presenting alternative to questionable allocations or alternative solutions to difficult problems, will act as a bridge between home care staff, clinical team members and other team members when assessing and monitoring home care service allocations and actively participates in the establishment of difficult or challenging home care service plan of care.
- Directs, facilitates and consistently evaluates job performance of the Community Care Supervisors, including managing duties, submitting payroll and evaluating performance. This is demonstrated by attending PCA meetings, meeting and talking with PCAs and team members, review of staff evaluations, field visits with CCS, disciplinary actions, grievances, payroll, expiration reports, training reports, review of CCS completed staff and ppt. assessments.
- Demonstrates the ability to effectively communicate with staff and other audiences in a clear, focused, non-defense and rational fashion, and adjust teaching/communication style to ensure clear understanding and reception of information and the meeting of teaching objectives and information exchanges.
- Participates as a member of the team for ongoing assessment, development and evaluation of the participant plan of care. Actively questions, educates and supports the team in the proper PACE model process of evaluating and allocation of PCA home Care services based on need. Will demonstrate an understanding of need vs want concept of PACE, will make decisions based on facts and assessment not third party reports only.
- Effectively participates in the on-call process while diligently evaluating all calls to impact improvements in services, outcomes and satisfaction of both participants and PCA staff. Will demonstrate an understanding of on call responsibility by being available for after hour calls and showing effective leadership in on call decisions and problem solving in collaboration with other on call staff.
- Establishes and maintains cooperative working relationships among staff. Facilitates and maintain a high standard of cohesive and collative team work within all home care department teams. Will show the ability to defuse conflict, to maintain focus on task at hand and identify area of improvement or action, to uphold professionalism and engage confidentially when appropriate.
- Provides services promptly, exercises tact, patience and courtesy at all times and conducts themselves in a professional manner with Participants, families, co-workers and any external contacts. As a representative of LIFE Pittsburgh, promotes goodwill and confidence in our Participants, staff, vendors, visitors and the general public.
- Proactively maintains requirements for the position (e.g., access of a vehicle, valid driver’s license and auto insurance, annual TB, bi-annual physical, etc) and maintains compliance with educational requirements (e.g., Fire Safety, Infection Control, Annual Training, Safe Mobility).
Working Conditions:
- Works within the LIFE Pittsburgh Adult Day Health Centers (ADHC), or occasional travel between centers for other related functions.
Required Certification/License:
- LPN or RN strongly preferred
- Valid Driver’s license and automobile insurance
- Must have a vehicle for travel
Education/Experience:
- High School Diploma or equivalent, Bachelor’s degree preferred
- Minimum of (1) year experience in a hospital, long-term care facility or community setting.
- Experience working with and relating to the geriatric population and their family members.
- Prior management/supervisory experience strongly preferred.
