Program Description: ProHealth is an integrated care clinic treating medical and mental health needs in one location, improving access to care for young adults ages 12 and older. ProHealth is a partnership between The Mental Health Center of Greater Manchester and Amoskeag Health offers wellness programs with tools and support. This model emphasizes integrated behavioral health and physical health care, timely access, coordination across systems, attention to social drivers of health, and team-based support for individuals with complex needs.

Requirements

  

Key Responsibilities:

· Manage a portion of the active ProHealth caseload in coordination with the existing case manager, medical assistant, and providers.

· Track provider-ordered follow-up needs, including lab work, x-rays, imaging, specialty referrals, and requested outside records.

· Monitor upcoming provider appointments to confirm whether required follow-up tasks have been completed before the visit.

· Provide outreach to clients who have pending follow-up needs, missed appointments, or repeated barriers to engagement.

· Coordinate transportation, appointment reminders, scheduling support, and resource connection to reduce DNA/no-show rates.

· Communicate referral status, outstanding follow-up needs, and client barriers to providers and care team members.

· Support clients in navigating insurance, benefits, community resources, social services, and external medical systems.

· Assist with transitions of care after hospitalization, emergency department use, crisis episodes, or higher levels of care.

· Maintain timely documentation of case management interventions, follow-up status, care coordination activities, and client progress.

· Participate in team huddles, care coordination meetings, and case consultation to support shared planning and accountability.


Qualifications:

· Experience in behavioral health, community mental health, integrated care, care coordination, case management, or human services.

· Ability to manage a complex caseload and track multiple follow-up needs across providers and external systems.

· Strong communication, organization, documentation, and problem-solving skills.

· Knowledge of social determinants of health, community resources, benefits, transportation supports, and healthcare navigation.

· Ability to collaborate effectively with providers, medical assistants, care coordinators, clinicians, external partners, and clients.

· Commitment to trauma-informed, person-centered, culturally responsive, and recovery-oriented care.


Experience: 

· Two or more years of experience in the community mental health field. 

· Preferred two years of clinical experience in an integrated primary and behavioral health care setting, or an equivalent combination of skills and experience.


Education: 

· Bachelor’s degree in social work, psychology, counseling psychology, or related clinical field is required
