Case management is a method of providing services whereby the case manager assesses the needs of the client and the client's family, when appropriate, and arranges, coordinates, monitors., evaluates, and advocates for a package of multiple services to meet the specific client's complex needs. 

Case Managers must be highly organized, empathetic, compassionate, able to work with diverse populations, and eager to help vulnerable adults. 


ESSENTIAL FUNCTIONS:

  1. Conduct client assessment and determine eligibility under program guidelines.
  2. Conducts home visits for initial, annual and health assessment. 
  3. Develop a case plan in accordance with client needs and implement plan by providing, arranging, and coordinating services.
  4. Maintain contact with client according to client’s needs and review plan periodically to ensure appropriateness thereof.
  5. Provide management of service delivery, contact referral services, and coordinate with other agencies to provide the client with services not met by OCOA programs. 
  6. Compile and maintain progress notes related to service provision and monitoring.
  7. Follow all policies and procedures of Osceola Council on Aging, Inc.                                                                                                      OTHER RELATED FUNCTIONS:                                                                                                                                                                       Hours of Work: Full-time 40 hours/weekly - office open 8:00 am to 5:00 pm, flex time with supervisor’s approval. May require some evening or weekend hours for special projects. Additionally, the employee must be able to work off-site at any of the locations in the planning and service area. 


Requirements

Must be fluent in both oral and written Spanish and English

   

REQUIREMENTS:

Background: Must Pass Level 2 background/security screen. 

Experience: One year of social service experience. Must know computer and word processing applications. Must possess a valid Florida driver’s license and reliable vehicle. 

Interpersonal Skills: Must have excellent interpersonal, communication, and organizational skills and the ability to work with a wide variety of groups and individuals. 

Essential Technical/Motor Skills Sufficient skills to be able to read and write effectively in English.

Salary Description
$18.00 to $20.00 per hour
