Under the direction of the Program Director is responsible for the overall data entry functions of the ARIES program. Responsible for providing ongoing assistance to Program Director provider case management services to clients who have been diagnosed with HIV/AIDS in Maverick, Val Verde, Uvalde, Dimmit, Zavala, Real, La Salle, Edwards and Kinney Counties. Responsibilities include but are not limited to, compiling client’s data, evaluating client’s needs and coordinating all social case management services with the Program Director. Responsible for the various functions required for administering the Ryan White, and Social Service. Responsible for demonstrating the knowledge and skills necessary to provide care appropriate to the age of the clients served, including demonstrating knowledge of the principles of growth and development over the life span. Must be able to identify and categorize each patient’s age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients. Will assist in the prevention of health education and disease prevention in the community and all counties served as needed and/or required. Responsible for promoting the “Health Promotion” program in the Tri County area. Responsible for assisting the CEO/CMO during the preparation and conduction of classes, campaigns and outreach activities. Serves as a health motivator offering patients in the program encouragement and support. Influences patients in the program to make lifestyle changes regarding diet and exercise. Performs various clerical functions ensuring the proper completion and handling of the required forms as well as tracking program participation and attendance. Responsible for compliance with other requirements affecting United Medical Centers, such as State/Federal regulations, PCHM Standards and Compliance Standards.

EDUCATION: High school graduate or equivalent. Graduate from an accredited training program for Medical Assistants and/or Community Health Worker. Certification or registration as Medical Assistant or Community Health Worker required.
EXPERIENCE/TRAINING: Computer experience in different programs required. Bilingual (English/ Spanish) required. Ability to communicate proficiently with individuals of diverse backgrounds and economic levels. Valid Texas Driver License and Liability Insurance coverage. Good driving record. May be required to stay on overnight travel, as necessary.
