Seniors At Home offers a wide range of services to help older adults live safe, healthy, and independent lives, including home care, personal assistant services, companionship, and specialized dementia and palliative care. We provide the Bay Area’s leading continuum of care for aging adults. 


POSITION SUMMARY:

Jewish Family and Children's Services (JFCS) is seeking a compassionate and experienced Temporary Dementia Specialist to join our Center for Dementia Care. This temporary position (duration of assignment is to be determined) provides an opportunity to support individuals living with dementia and their families through comprehensive assessments, care planning, education, and ongoing care management while contributing to one of the Bay Area's leading nonprofit organizations.


COMPENSATION AND BENEFITS:

  • Salary Range: $75,000 - $80,000 (depending on experience)

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Conduct comprehensive dementia assessments and develop individualized care plans
  • Provide care management and ongoing support to clients and family caregivers
  • Educate and coach families and caregivers using evidence-based dementia care practices
  • Collaborate with physicians, community providers, and interdisciplinary team members to coordinate services
  • Train caregivers in effective communication techniques and non-pharmacological dementia interventions
  • Assist with dementia-related education and training for agency staff
  • Make appropriate community referrals and connect clients with supportive resources
  • Participate in outreach activities that promote dementia education and JFCS services
  • Maintain accurate documentation and ensure high-quality client care

JOB QUALIFICATIONS:

  • Bachelor's degree required; Master's degree in Social Work, Gerontology, Nursing, or a related field preferred
  • Minimum three (3) years of professional experience working with individuals living with dementia and their families
  • Strong interpersonal, assessment, and communication skills
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Valid California driver's license, reliable vehicle, and ability to travel throughout the Bay Area
