Summary:
Supports federally regulated Medicaid case management services under Title XIX programs in compliance with federal Medicaid rules, Washington Administrative Code (WAC), and agency policy. Responsible for assisting with comprehensive CARE assessments, authorizing long-term care services, issuing Planned Action Notices (PANs), and ensuring regulatory compliance. Assists in assessment of client needs, assists in development and monitoring of service plans, makes appropriate referrals and serves as a customer advocate with other service providers. Assists with division day-to-day operations as delegated, including reports, outreach, community events, support groups, and more. Requires knowledge of related community resources and the laws and regulations specific to the area of service

Essential Functions:
- Assist case managers in completing the initial interviews with the customer and his or her family to assess the customer's eligibility for services.
- Assist case managers in gathering relevant and useful data from the customer, family, other agencies, to formulate a psychosocial assessment of the customer and his or her family.
- Assist case managers in monitoring adherence to the plan and managing the flow of accurate information within the action system to maintain a goal orientation and coordination momentum.
- Provide "follow-along" to the customer and his or her family to speed up identification of unexpected problems in service delivery and to serve as a general troubleshooter on behalf of the customer.
- Assist case managers in completing the necessary paperwork to maintain documentation of customer progress and adherence to the plan by all concerned.
- Act as a liaison between programs, providing services to the customer to ensure the smooth flow of information and minimize conflict between the subsystems.

Required Education or Experience:
High School Diploma and 1 year demonstrated experience in social services, including case management

Any combination of education and experience would provide the applicant with the desired skills, knowledge, and ability required to perform the job.
Required Certification and Licenses:
• Valid driver’s license in state of residence.
• Auto insurance in the amount required by the State of Washington.
• Acceptable completion of a criminal history background check

Additional wage progression may be available over time under the organization’s established compensation structure. The full established pay scale for this position is $19.87 - $ 52.96 per hour; however, placement above the starting wage range is not available at hire.
