Job Overview:
The Jordan Valley Senior Care (JVSC) Development Coordinator is responsible for growing and sustaining participant enrollment in the program by building strong referral relationships throughout the community. This role serves as the primary liaison to skilled nursing facilities (SNFs), hospitals, senior centers, and other community partners, and leads internal program education efforts within Jordan Valley Health. The Development Coordinator also serves as a trained backup to the Intake/Enrollment Coordinator, ensuring continuity of the enrollment process during absences or periods of high volume.
Key Responsibilities:
Community Outreach & Referral Development
- Identify, cultivate, and maintain referral relationships with skilled nursing facilities, hospitals, discharge planners, senior centers, physician offices, and other community-based organizations serving older adults.
- Develop and execute a proactive outreach plan to generate program referrals and grow enrollment within the program's service area.
- Represent JVSC at community events, health fairs, senior center presentations, and provider meetings.
- Track outreach activity, referral sources, and conversion outcomes; report trends and opportunities to program leadership.
- Serve as the primary point of contact for referral partners, responding promptly to inquiries and following up on referral status in collaboration with the Intake Coordinator.
- Support the development of marketing and educational materials, in coordination with program leadership, to facilitate referral partner and community education.
Internal Education & Promotion
- Promote awareness and understanding of the JVSC program among Jordan Valley Health staff, providers, and departments.
- Develop and deliver internal presentations, trainings, and materials to help staff identify appropriate JVSC referral candidates.
- Serve as an internal subject-matter resource on program eligibility, enrollment, and the scope of services provided.
- Coordinate with clinical and administrative teams to strengthen internal referral pathways into JVSC.
- Promotes the education and development of students, interns, residents, apprentices, and other new staff by sharing expertise, responding to questions, and fostering a positive and supportive learning environment.
Intake/Enrollment Support
- Train on and maintain working knowledge of the full intake and enrollment process, including eligibility screening, application processing, and required documentation.
- Serve as backup coverage for the Intake/Enrollment Coordinator during absences, vacations, or periods of high referral volume.
- Support timely follow-up with prospective participants and families throughout the enrollment process as needed.
- Ensure enrollment activities remain compliant with program, state, and federal requirements while providing coverage.
Regulatory Requirements
- Only act within the scope of his/her authority to practice.
- Meet a standardized set of competencies for the specific position description established by Jordan Valley Senior Care and approved by regulatory agencies before working independently.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
- Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Required Qualifications:
- 2+ years of experience in community outreach, healthcare business development, referral relations, or intake/enrollment, preferably in a senior care, PACE, or long-term care setting.
- Either one year of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from the JVSC on working with a frail or elderly population upon hire.
- Basic Life Support (BLS) required, must obtain within first 90 days of employment
- Valid driver's license and reliable transportation.
Preferred Qualifications:
- Bachelor's degree in healthcare administration, social work, marketing, public health, or a related field preferred; equivalent experience considered.
- PACE experience preferred.
