Position Description – What the Role Entails:
The Case Manager functions as a member of a multidisciplinary behavioral health team, providing community-based case management, care coordination, and rehabilitative services to individuals enrolled in the CDPH Mental Health Expansion Grant. The Case Manager works collaboratively with clients and their treatment team to address social determinants of health and improve access to behavioral health and community resources. Responsibilities include linking clients to mental health, primary care, substance use treatment, housing, employment, education, transportation, financial assistance, and other supportive services; assisting with benefits and entitlement applications; monitoring progress toward individualized goals; conducting outreach and engagement; coordinating care across providers; and advocating for clients to reduce barriers to care. Services are primarily delivered in the community, meeting clients where they are, and require ongoing collaboration with healthcare providers, community organizations, and other grant partners to promote recovery, stability, and overall wellness.

Areas of Accountability - What you will be doing:
Assessment, Evaluations & Treatment Planning
- Participate in the intake and engagement process by gathering psychosocial, functional, and social determinants of health information to support the clinical assessment.
- Complete required screening and assessment tools within the scope of practice, including IM+CANS (certification required), and document findings according to agency, Medicaid, and grant requirements.
- Identify client strengths, needs, barriers to care, and risk factors, promptly communicating significant concerns to the supervising clinician and treatment team.
- Collaborate with clients, natural supports, and the interdisciplinary team to develop, implement, and monitor individualized care coordination goals aligned with the treatment plan.
- Review client progress and update care coordination goals as client needs change, participating in interdisciplinary case reviews as needed.
Service Delivery
- Provide community-based behavioral health case management and care coordination services in office, home, hospital, or other community settings.
- Connect clients to behavioral health, primary care, substance use treatment, housing, employment, education, transportation, financial assistance, and other community resources to address identified needs.
- Assist clients with obtaining and maintaining public benefits and entitlements, including Medicaid, SNAP, SSI/SSDI, and other eligible programs.
- Monitor service engagement, address barriers to care, and coordinate with internal and external providers to promote continuity of services.
- Provide supportive interventions, skill building, advocacy, and psychoeducation that promote recovery, independence, and community integration.
Crisis Response
- Recognize behavioral health crises and respond within the scope of practice by utilizing de-escalation techniques and coordinating with licensed clinicians or emergency services as appropriate.
- Reinforce safety plans, monitor ongoing risk, and communicate changes in client status to the treatment team.
- Provide supportive case management services that promote client stability and recovery.
Community Engagement
- Conduct outreach and engagement activities to identify and connect eligible individuals with behavioral health services.
- Develop and maintain collaborative relationships with healthcare providers, community organizations, and social service agencies to improve access to care.
- Advocate for clients by reducing barriers to services and promoting access to community supports.
- Document outreach and community engagement activities as required.
Supervision and Oversight
- Participate in regular supervision with an RFS Licensed Practitioner of the Healing Arts (LPHA), with a minimum of one documented supervision session each month.
- Seek consultation for clinical, ethical, or safety concerns outside the scope of practice.
- Deliver services in accordance with agency policies, grant requirements, Medicaid regulations, and applicable state and federal standards.
Documentation and Compliance
- Maintain timely, accurate, and compliant documentation in accordance with grant requirements, agency policies, Medicaid regulations, and applicable licensing and accreditation standards.
- Services are to be documented in the clients’ files within 48hrs of service delivery.
- Maintain client records that meet documentation standards of the agency and all regulatory standards.
- Participate in quality assurance and quality improvement activities as directed.
- Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Corporate Compliance policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EHR access. This includes immediate reporting of any PHI breach of Corporate Compliance or outside provider's health records to the Corporate Compliance Legal and Quality departments, as well as to the program administrator.
Professional Development
- Complete all agency and other regulatory body required trainings, including topics like state administrative rules, sexual harassment, fraud waste and abuse, mandated reporter, clinical courses, diagnosis and other requirements.
- Complete all training required to keep any licenses or certifications relevant to your position.

Rincon’s Work Environment – What to expect:
Physical Demands: The MHP must be able to travel. This work is primarily in the community, either in responding to hospital discharges or in outreach and engagement activities. - This position is scheduled to be available from 09:00 am to 05:00 pm, Monday through Friday.

Position Qualifications – What is required to Perform the Role:
- Bachelor’s degree in social service field or more than 5 years of experience.
- IM+CANS certified preferred.
- As evidenced by previous work experience, academic background, and in-service or other training, the MHP shall demonstrate a strong understanding of how to:
- Identify precipitating events.
- Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts.
- Assess family, social and work relationships.
- Conduct and document an integrated mental health assessment.
- Utilize appropriate additional diagnostic tools (e.g. LOCUS, ASAM criteria...).
- Under supervision, recommend a DSM diagnosis.
- Under supervision, write and implement an integrated treatment plan.
- Under supervision, and within the scope of one’s practice, provide mental health services.
- Must pass pre-employment background checks. This includes clearance by the DHS Background Check Unit.
- Full time use of a properly licensed and insured automobile with a valid driver’s license and ability to drive, or independence in travel arrangements.
- Must adhere to the agency’s non-discrimination policies.
