The Youth Trauma Counselor & Case Management Specialist is responsible for providing trauma-informed mental health services, case management, survivor support programming, and family engagement services for youth impacted by gun violence. This position collaborates closely with school administration, caregivers, community partners, and program stakeholders to improve mental health outcomes, strengthen resiliency, and increase access to supportive services.

The position delivers individual and group counseling, coordinates case management services, facilitates youth and caregiver workshops, supports program evaluation efforts, and contributes to ongoing outreach and sustainability initiatives

Major Duties and Responsibilities

• Provide weekly individual counseling services utilizing evidence-based and trauma-informed approaches.

• Develop individualized treatment plans and case goals.

• Monitor participant progress and adjust interventions as needed.

• Engage caregivers and family members in counseling services when appropriate.

• Conduct participant intake assessments and ongoing evaluations.

• Maintain confidential and accurate clinical documentation.

• Participate in weekly planning meetings and incident response meetings with school leadership and staff.

• Coordinate program scheduling and implementation activities within the school setting.

• Assist with curriculum development and adaptation of youth-focused programming.

• Support student recruitment and participant engagement efforts.

• Collaborate with school administration to ensure program goals and services align with student needs

• Administer participant intake questionnaires and assessments.

• Collect, track, and enter participant data in compliance with grant and reporting requirements.

• Conduct pre/post assessments and participant satisfaction surveys.

• Monitor outcome measures and prepare periodic data reviews and reports.

• Support program evaluation and continuous quality improvement efforts.

• Assist in preparing final program reports and documentation.

Education

 • Preferred Master's degree in Social Work, Counseling, Psychology, Mental Health Counseling, or a related behavioral health field

Experience

• Minimum 2 years of experience providing counseling, case management, or behavioral health services to youth.

• Experience facilitating psychoeducational and support groups.

• Experience working with trauma-affected populations.

• Experience collaborating with schools, families, and community organizations.

• Experience working with survivors of violence or victim service programs.

• Familiarity with Cognitive Behavioral Therapy and trauma-informed care models.

• Experience with grant-funded programs and outcome measurement

• Strong understanding of trauma, violence prevention, and adolescent development.

• Excellent counseling, crisis intervention, and de-escalation skills.

• Ability to facilitate group discussions and educational workshops.

• Strong case management and resource coordination skills.

• Excellent written, verbal, and interpersonal communication skills.

• Strong organizational and data management abilities.

• Ability to work effectively with diverse youth, families, school staff, and community stakeholders.

CONDITIONS OF EMPLOYMENT

• Ability to work varying hours Monday - Friday. Schedule may include some evenings and weekends. 
• Must have a valid driver’s license and automobile insurance required for work related travel

PHYSICAL REQUIREMENTS 

• Must be able to remain stationary 50% of the time.
• Must be able to frequently move inside the office to access file cabinets, office machinery, etc. 
• Must be able to transport oneself to and from home visits.
• Occasionally ascends/descends stairs.
• Constantly moves to assist during program activities.
• At times, it may be required to lift up to 25 lbs. for various needs. 
• Frequently moves in outdoor weather conditions.
